Knowledge Skills and Abilities
- Doctor of Pharmacy (Pharm.D.) from an accredited College/School of Pharmacy.
- Exercises independent judgment and decisionmaking skills.
- Ability to coordinate the sequence of an operation and/or process.
- Relate cooperatively & constructively with patients staff clinical/medical staff & interdisciplinary teams.
- Knowledge of pharmacokinetic and pharmacotherapeutic properties of medications.
- Provide oncall afterhours.
Job Summary
Plans organizes supports and maintains clinical pharmacy programs and services to support patient care and professional standards of practice. Recommends innovations in clinical pharmacy practice and function of the hospital pharmacy. Involved in training and education for pharmacy students and medical residents. Provides clinical support for pharmacists technicians hospital and medical staff in NICU Oncology Critical Care and any other areas as assigned. Interprets medication orders ensures accurate medication order processing dispensing preparation and application of clinical knowledge to meet all state and federal laws and regulations. Works under the general supervision of the Director Clinical Residency Director and clinical supervisory staff.
Job Specific Duties
- Delivers quality pharmaceutical care and effective medication management outcomes through error free care timely verification and delivery of medications.
- Participates in hospital based Code Blue events medication reconciliation and providing evidenced based drug information.
- Maintains competency with safe use of pharmacy and hospital clinical information systems (Cerner Pyxis C2 Safe Baxa TPN).
- Ensures appropriate aseptic IV technique to meet USP 797 standards as required and completes annual IV Room competency training and testing.
- Ensures working clinical knowledge and practice skills related to TPN Chemotherapy and Investigational Drug processing preparation and dispensing.
- Directs and supervises technical staff related to daily medication compounding preparation and verifies accuracy of medications prior to dispensing.
- Interprets and processes medication orders timely accurately and manages medication use based on state and federal regulations.
- Performs quality review to achieve metrics & document interventions clinical activities adverse drug reactions and medication errors to meet TJC state & federal requirements.
- Screens for adverse events such as allergies correct dosage incompatibilities duplications adverse drug reactions contraindications drugdrug & drugfood interactions.
- Weighs measures and prepare oral and IV medication products utilizing appropriate tools and equipment.
- Responsible for advances in the profession through publication presentations at local state or national meetings and providing community education.
- Attends & contributes daily patient care rounds clinical services meetings committees task force projects & provides education participates in department inservice programs.
- Coordinate educate and maintain investigational drug study activity specific to their area of practice and dispenses investigational drugs where needed.
- Develop and implement treatment guidelines protocols formulary changes and clinical pathways as approved by appropriate committees.
- Document quality activities to achieve establish metrics for TJC state & federal requirements.
- MUE data collection antibiotic sensitivity adverse drug reaction med error intervention reports drug monographsP&T & newsletter for Antibiotic committee dept. & hospital QA.
- Assess patient care conditions and evaluate patient response to medication therapy regimens.
- Communicates and presents information effectively in both oral and written form with all levels of hospital personnel and all patient groups.
