You must provide the following certifications:

1. Current CA State EMT Certification
2. AHA CPR/BLS Certification
3. Current California Drivers License
4. Ambulance Drivers License
5. Medical Examiners Certificate
6. DMV K4 Motor Vehicle Report (currently accepting online printout from DMV)
