Shift: Day Shift length: 8 HoursPOSITION SUMMARY:
The Oncology Nurse Navigator has responsibility for coordinating the care and services for oncology patients to ensure that quality multidisciplinary care is provided to all patients seen while assuring a high level of customer service. Navigation services include prevention and screening activities and continue through diagnosis treatment survivorship and endoflife care. The Oncology Nurse Navigator utilizes advanced clinical skills to facilitate the provision of care throughout the healthcare continuum which includes the assessment planning intervention (strong patient/family education component) and evaluation principles of the nursing process. The navigator will work closely with patients providers and other health care professionals to provide support around treatment adherence and selfmanagement identifying and addressing needs in care as appropriate and serves as a resource for clinical staff and recognizes when intervention is needed. They serve as a patient advocate promoting effective utilization of resources using clinical knowledge awareness of community services and assuming a leadership role with the patient care team to achieve optimal clinical and resource outcomes through the health care system. The Oncology Nurse Navigator determines the needs of the newly diagnosed oncology patients and facilitates access to appropriate resources in a timely manner. The Oncology Nurse Navigator adheres to established clinical policies protocols regulations and standards. The Oncology Nurse Navigator assists in transitioning patients to survivorship and works in collaboration with the providers to create Survivorship Plans and Care Summaries as appropriate.

SKILLS / KNOWLEDGE / ABILITIES:
Assessment planning intervention and evaluation of patient care. Critical thinking and analytical skills. Must have excellent communication and interpersonal skills to establish a high degree of rapport and professional interaction. Ability to build positive working relationships with physicians and external agencies. Ability to communicate with multiple levels of the organization (e.g. managers physicians clinical and support staff). Capable of using (Microsoft Office Suite) Microsoft Word Excel Outlook Midas and EPIC. Organizational and multitasking skills are essential. Working knowledge of community resources. Ability to prioritize and reprioritize quickly. Ability to work in teams as well as autonomously. Must demonstrate technical competence communication skills and sensitivity for the family in crisis. Furthers education and clinical skills by attending classes and workshops. Must be able to fulfill the essential functions of the position.
