Jefferson Healthcare a leading and awardwinning healthcare system is seeking a fulltime Nurse Practitioner or Physician Assistant to join our Palliative Care Team. This position primarily involves providing homebased care with some clinic visits and occasional inpatient consultations. It offers a unique opportunity to make a meaningful impact in a closeknit rural community. Were looking for a compassionate and experienced provider who excels both independently and as part of a team with expertise in Palliative Care Home Health and Hospice services.
The ideal candidate will be Board Certified/Board Eligible have experience in both inpatient and outpatient settings should be comfortable with geriatrics and a wide range of clinical conditions and be motivated to live in a rural area while delivering patientcentered care.
