Job Summary

The PostGraduate Year 1 (PGY1) Pharmacy Practice Residency program is a 12month postgraduate training program which builds on the Doctor of Pharmacy (PharmD) education and outcomes to contribute to the development of clinical pharmacists responsible for medicationrelated care of patients with a wide range of conditions eligible for board certification and eligible for postgraduate year two (PGY2) pharmacy residency training. The resident actively participates in a multidisciplinary care environment. Completes the learning experience through fourtofiveweek rotations in differing clinical specialty areas as well as longitudinal learning opportunities in ambulatory care drug information and administration. In general the resident will be expected to learn from their preceptors and apply the lessons learned to direct patient care activities. A major and minor project will be assigned and completed that will improve the overall status of pharmaceutical care of our patients at Denver Health. Residents are taught and evaluated by the educational goals and objectives set forth by the American Society of HealthSystem Pharmacists (ASHP) Accreditation Board.

Essential Functions:
  • Designs therapeutic drug regimens or plans based on patientspecific information that includes establishing a therapeutic endpoint selecting appropriate drug and related therapy and establishing an appropriate dosing regimen. Demonstrates a sound problem solving/decision making skills with practical efficient economical and reasonable solutions. (20%)
  • Monitors drug therapy for potential drugdrug drugfood or drugdisease state interactions. Assess and monitor patient response to medication regimens documents all patient activities and interventions in the patients electronic health record. Demonstrates accuracy thoroughness and attention to detail. (20%)
  • Establishes a presence and working relationship with providers/staff in practice area of each rotation. Attends patient care rounds as applicable to clinical specialty area. Collaborates with formulary or preferred agents evidencebased medicine drug information to both providers and patients drug interaction screening medication profile review assessment of patient response to therapy. (20%)
  • Assists in the formulary management process at Denver Health by preparing new drug monographs conducting therapeutic drug class or disease state reviews development of clinical practice guidelines or development of other policies and procedures. Participates in the education and implementation of new formulary guidelines for providers/staff as assigned. Attends P&T Committee or other medication management committee meetings throughout the year. (10%)
  • Learns with Lean tools and department quality improvement projects. Participates in a Lean event and department goal setting sessions. Reports and follows up on medication/patient safety issues in area of practice. Learns with patient safety goals or core measures that affect the pharmacy department and participate in development of programs that ensure compliance with these measures. (10%)
  • Participates in and provides educational programs for the department or personnel as required by specific rotation preceptors. Completes an inservice presentation for pharmacy staff during each rotation. Assist in the development of competency modules for staff as assigned. Presents results of residency project at the regional residency conference. Prepares manuscript of major project prior to completion of residency. Provides education and training by serving as a preceptor for pharmacy interns and students on rotation at Denver Health. (10%)
  • Attends and participates in departmental and/or systemwide meetings inservices huddles and committees. Completes selfassessment of current practice goals and longterm goals annually. Identifies opportunities for learning and training to advance. Receives a passing score on all evaluations for individual rotations throughout the residency year. (10%)

Education:
  • Bachelors Degree BPharm and DORA CO Pharmacist License within first 4 months of residency Required or
  • Bachelors Degree PharmD and DORA CO Pharmacist License within first 4 months of residency Required

Work Experience:
  • 13 years pharmacy intern in healthsystem setting experience preferred

Licenses:
  • BLSBasic Life Support (BLS/CPR) AHA American Heart Association
  • ACLSAdvanced Cardiac Life Support AHA American Heart Association
  • PALSPediatric Advanced Life Support AHA American Heart Association
  • PBIDPharmacy Based Immunization Delivery BPS Board of Pharmacy Specialties

Knowledge Skills and Abilities:
  • In depth knowledge of current evidencebased practice drug information and drug interactions.
  • Strong communication and active listening skills.
  • Ability to be conscientious and pay close attention to detail.
  • Good time management and organizational skills are beneficial to success during the residency program.
  • Proficiency with Microsoft Office required.
  • Proficiency with electronic health record software Epic preferred.
