About PACE:
Philanthropists Gary and Mary West founded West Health with the mission of enabling seniors to successfully age in place with access to highquality affordable health and support services that preserve and protect their dignity quality of life and independence. The Gary and Mary West PACE (Program of Allinclusive Care for the Elderly) affiliated with West Health () is a new nonprofit that delivers medical social and behavioral services to the frail elderly to the North County of San Diego. This PACE center will be entrepreneurial in nature and will focus on innovations and bringing new processes to the traditional PACE environment.

Position Summary:
The Scheduler will coordinate and schedule appointments for all participants of Gary and Mary West PACE. This position will prepare schedules according to the participants needs and the participants care plan designed by the interdisciplinary team. The Scheduler will work closely with the transportation department PACE participants caregivers and the Center staff.
Essential Functions:
Create and maintain all schedules for PACE care team according to participant care needs.
Input return visit requests and semiannual assessments into schedule.
Coordinate participants medical appointment schedules with all departments within PACE.
Maintain accurate records of all scheduled appointments cancellations and changes.
Ensure all appointments are scheduled in a manner that promotes participant satisfaction and convenience.
Serve as the primary point of contact for families caregivers and all disciplines to ensure patient care is uninterrupted and wellcoordinated.
Process and manage all transportation requests for participants attending appointments at the Center.
Work with transportation teams or vendors to ensure that participants have appropriate travel arrangements for their appointments.
Monitor and resolve scheduling conflicts to prevent doublebooking or overlaps.
Ensure participant education has been completed by department prior to appointments.
Create electronic health record (HER) authorization transportation appointment escort appointment etc.
Confirm appointments with participants and families and address any concerns.
Distribute schedules and other necessary information to the appropriate staff and/or participant.
Interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties.
Participate in required staff and training meetings.
Maintain strict confidentiality of all participant information.
Attend and participate in all staff meetings inservices special projects and committees as assigned.
Adhere to and support the centers policies practices and procedures.
Accept assigned duties in a cooperative manner; and perform all other related duties as assigned.
Be flexible in schedule of hours worked.
Education and Certification:
High School diploma or GED required.
Skills & Experience:
Must be able to demonstrate proficiency of Microsoft computer applications. Experience working with Electronic Medical Record system (EMR) preferred.
Minimum of one (1) year of documented experience working with a frail or elderly population required.
Skilled in communicating with participants their families and other staff members; ability to establish a helping trusting relationship with participants.
Ability to follow through with instructions and directions provided by supervisor RN QAPI coordinator primary care physician and/or medical director.
Ability to utilize appropriate safety measures when providing care.
Ability to work effectively and harmoniously as part of an interdisciplinary team the elderly providers of services and peers.
Well organized dependable flexible and resourceful.
Medical Clearance:
Employees must have medical clearance for communicable diseases and uptodate immunizations before having direct participant contact. Contingent upon clear Background check.
