  • Pay Rate: $29.00 per hour
  • Position Status: Fulltime 40 hours per week
  • Schedule: Monday Friday 8:30 am 4:30 pm

Responsibilities

Heres what to expect as a Program Specialist:

  • Evaluate referrals to determine appropriateness of placement and work collaboratively with the Residential Manager and the Residential Director to assess the ability to support individuals referred to residential services.
  • Develop annual Personal Support Plans that meet the specific needs and desires of each individual supported in collaboration with all team members.
  • Monitor the implementation and the effectiveness of Personal Support Plans/Individuals support plans.
  • Provide the assistance training and support necessary to assist the individual to develop and increase personal competencies and become socially integrated into their communities.
  • Conduct weekly audits of files of residents on caseload.
  • Communicate results of audits to Residential Manager Residential Supervisor and the Residential Services Director.
  • Develop and implement new policies and/or procedures within the functional area to support the continuous improvement of operational effectiveness and efficiency.
  • Complete all assessments and quarterly reports within the required timeframes.
  • Ensure compliance with applicable licensing regulations.

Qualifications

Minimum requirements as a Program Specialist:

  • Associates or 60 credit hours from an accredited college or university required and 4 years of prior work experience working directly with persons with disabilities.
  • OR Bachelors degree from an accredited college or university required and 2 years of prior work experience working directly with persons with disabilities.
  • A valid drivers license.
  • Access to vehicle with valid insurance registration and inspection.
  • Ability to communicate read write and interpret English.
  • Flexible to work all assigned work schedules including holidays evenings nights and weekend hours as business needs dictate.

Preferred but not required:

  • Knowledge of applicable compliance regulations (i.e. Chapter 51 6400.
  • Developmental Planning and Goal Setting experience.
  • MultiSite Management experience.
  • Knowledge of disability services funded through Medicaid and through PAs Department of Health and Human Services.
