UPMC Central PA PostAcute team provides care in several skilled nursing facilities across the Care physicians hospitalists and geriatric trained physicians are encouraged to the Position:Provides direct patient care in multiple facilities across the aging continuum to include skilled nursing rehab personal care/assisted living independent living and with facility leadership hospital discharge team care management and other specialties to drive optimal quality care with aligned care offers a wide variety of healthcare fields from general primary care and internal medicine to geriatrics and acute rehab services. Our clinicians crossover and work with several specialists increasing their knowledge base and with and supervises Advanced Practice EMR online diagnostics including PACS imaging and laboratory rounding MF attractive daily schedules with flexible models.
