Job Description
Methodist Hospital is looking to hire a Social Work Care Manager to join our Care Integration team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.
Position Summary:
The Social Work Care Manager provides patientcentered care management services to patients and families in an inpatient hospital setting. This position serves as an integral member of the interdisciplinary healthcare team andis responsible for identifying care coordination needs in collaboration with the inpatient and ambulatory care teams along with the patient and the patients external support systems. Within the context of an interdisciplinary team the Social Work Care Manager participates in progressing patients toward discharge/next level of care by identifying barriers implementing appropriate interventions and securing transition plans that are aligned with shared goals of Social Work Care Manager will consistently and thoughtfully apply social work values and continuous quality improvement in daily work.
Work Schedule:
- Monday Friday 84:30.
- Rotating weekends.
Required Qualifications:
- MSW Masters of Social Work
- Prefer a minimum of one year experience in a medical or communitysetting practicing social work
- Minnesota license to practice as LGSW or LISW
- Knowledge of the values principles and methodologies of social work
- Exceptional organizational and time management skills
