Clinical Medication Access Pharmacist AmbulatoryJob Description:
Job Summary: The Clinical Pharmacist provides ambulatory pharmaceutical care services using approved guidelines in collaboration with other health care professionals including managing patient drug therapy providing patient education and preparing/overseeing the preparation and dispensing of medications. Promotes high quality evidencebased and costeffective pharmaceutical care. Performs these duties within primary care and/or specialty clinics. This position will be working Monday to Friday working the day shift.
Essential Responsibilities:
Practices pharmacy in accordance with laws rules and regulation of the Massachusetts Board of Registration in Pharmacy Department of Public Health U.S. Food and Drug Administration and Drug Enforcement Administration and in accordance with established departmental policies and procedures. Maintains credentials in accordance with the Massachusetts Board of Registration in Pharmacy
Responsible for the daily operations of assigned areas. Manages workload and coordinates personnel in teamwork within the environment. Resolves operational problems arising in specific area. Assists in the development and maintenance of pharmacy policies and procedures and provide cares as described in the departments polices/procedures.
Evaluates medication orders; reviews disease state parameters allergies drugdrug interactions and duplication of therapy. Communicates with physician on discrepancies possible interactions or changes necessary. Informs nurse of problem and resolution. Documents all interventions.
Provides drug information to physicians nurses and other health care professionals. Continually evaluates quality and timeliness of dispensing performance within procedural framework. Uses initiative and resourcefulness to assure best possible patient service. Communicates applicable information to subsequent shifts ensuring the continuity of care.
Perform medication reconciliation at clinic visits to determine appropriateness of medication orders and to assess patient adherence.
Identifies resolves and prevents potential and actual medicationrelated problems including: untreated indications improper drug selection sub therapeutic dosage failure to receive medication adverse drug reactions drug interactions and medication without indication.
Coordinates continuing education activities for medical and nursing staff; orients new employees on pharmacy practices and procedures. Includes submissions to the pharmacy newsletters participating in journal clubs and providing inservices to professional staff.
Precepts PGY1 and PGY2 pharmacy residents pharmacy students and contributes to staff development in area of specialty practice. Works collaboratively with other clinical pharmacy specialists to develop the pharmacy skills of pharmacy staff members by teaching and exemplifying those skills to the team members.
Required Qualifications:
Bachelors degree in Pharmacy required.
License Massachusetts Pharmacist required.
13 years related work experience required.
Understanding of insurance and adjudication processes.
Experience with computer systems required including web based applications and some Microsoft Office applications which may include Outlook Word Excel PowerPoint or Access.
Preferred Qualifications:
Pharmacy residency PGY1/PGY2 specialty experience or equivalent experience or 35 years of clinical pharmacy experience.
Current Certified Specialty Pharmacist (CSP) board certification in pharmacotherapy (BCPS) Ambulatory Care Pharmacy (BCACP) or respective specialty and must obtain certification within 18 months of eligibility.
Knowledge of complex/chronic diseases states and therapies used within specialty pharmacy such as but not limited to: Primary Care Oncology Transplant Infectious Disease (including HIV) Multiple Sclerosis Rheumatoid Arthritis Gastroenterology Hepatology Dermatology Cardiology Pulmonary Allergy and/or various inflammatory conditions preferred.

Physical Nature of the Job:
