Job Description

Position: Mental Health Clinician II
Program: Permanent Housing Advanced Clinical Services (PHACS)
Location: 205 13th St. San Francisco
Reports To: Director of Clinical Services
FLSA Status: NonExempt Union
Wage: $40.00/hour

Are you interested in providing community mental health services to those in need We are a small dedicated team of Clinical Supervisors Clinicians Case Managers and Harm Reduction Specialists seeking to add a compassionate committed and driven person to our team! The ideal candidate is passionate about community mental health work and strives to make a difference in the lives of those who are marginalized underserved and most in need.

The Clinician II role is appropriate for a Masters level graduate who is registered with the BBS and collecting single adult hours towards licensure.

The current position available is in our new Permanent Housing Advanced Clinical Services (PHACS) program. In this role you will provide a blend of therapist/counselor and case management services to clients in permanent supportive housing (PSH) sites who are living independently and may be struggling to do so in healthy ways. You will work closely with your teammates including case managers and harm reduction specialists to provide traumaresponsive wraparound care for your clients. PHACS team members also provide consultation coaching and training to the onsite contracted providers in the PSH buildings where clients reside in an effort to improve access to care and to bridge services into ongoing solutions and connections to longerterm service support.

PHACS is a fieldbased program in which team members visit clients in their homes and accompany clients to appointments. The majority of clients live in the Tenderloin SOMA and Mission neighborhoods. Most team members walk and/or access public transportation when completing field work.

The ideal candidate for the position has a significant capacity for compassion for others as well as an established practice of selfcare and healthy boundaries.

ESSENTIAL JOB DUTIES:

  • Assess client needs via review of reason for referral and medical record direct client interview collaboration with existing treatment and care providers. Analyze data obtained to make recommendations for case disposition.
  • Develop and implement a comprehensive care plan aimed at stabilization wellness and recovery by engaging clients in appropriate treatment modalities and referrals for additional support as needed.
  • Facilitate referrals to appropriate community resources; actively manages caseload by maximizing use of community resources to meet client needs.
  • Compile and maintain client records by preparing written/electronic documentation including periodic reports forms and billing information to ensure proper compliance with legal provisions including mandatory reporting.
  • Establish and maintain professional relationships by collaborating and consulting with other service providers (e.g. nurses primary care providers with probation lawyers physicians) to consult on medical psychiatric and psychosocial issues.
  • Participate in and contribute to Quality Improvement activities
  • Engagement in care coordination and clinical supervision sessions.
  • Adhere to established policies and procedures.
  • Attend/participate in organizational trainings and meetings as assigned.
  • Other duties as assigned.

REQUIRED QUALIFICATIONS:

  • Completion of Masters level program in social work psychology mental health or counseling.
  • Ability to work sensitively with individuals from diverse cultures languages abilities backgrounds and identities.
  • Must have CPR and First Aid certification within 90 days of hire.
  • TB clearance and proof of COVID19 vaccination within 7 days of hire

DESIRED QUALIFICATIONS:

  • Registration and assigned intern/associate number with either Board of Behavioral Science (BBS) in the State of California; registration in good standing.
  • Proficient in documentation utilizing strong clinical language skills; knowledge and familiarity with San Francisco billing database (EPIC).
  • A strong understanding of the interconnection between complex medical conditions substance use disorder and mental health disorders.
  • Previous experience working with the populations and neighborhoods served.
  • Good computer skills in MS Office and GSuite applications and the ability to work in a networked computer environment.
  • Bicultural bilingual Spanish speaking or other SF Threshold Language strongly preferred
  • Knowledge of San Francisco neighborhoods and community resources.
  • Ability to effectively deescalate conflict
  • Knowledge of homelessness trauma domestic/intimate partner violence substance use disorder and recovery HIV/AIDS harm reduction mental health disorders neurodiversity LGBTQIA communities and systems of oppression.

PHYSICAL REQUIREMENTS:

  • Ability to stand and walk as needed throughout the shift for extended periods of time.
  • Ability to lift up to 50 lbs.
  • Ability to sit and use a computer including keyboard and visual display terminal for extended periods of time.
  • Finger hand and wrist agility.
  • Ability to complete tasks in an environment where interruptions are the norm and background noise is present.
  • Ability to move through buildings commonly including stairs between buildings and out in the community.
