We are seeking an Advanced Practice Provider to join one of the nations most integrated healthcare systems with over 1300 providers in Indianapolis IN. This opportunity is located at our Community Heart and Vascular Hospital at 8075 North Shadeland Avenue Indianapolis IN 46256
Highlights
Community Physician Network is seeking a boardcertified advanced practice provider to join a wellestablished vascular team at our Community Hospital East clinic. We are seeking a highly motivated experienced Physician Assistant or Nurse Practitioner or new graduate with experience in vascular surgery cardiology ER PCU or ICU firmly required.
- Vascular clinic and rounding responsibilities at Community Hospital East with crosscoverage at Community Hospital South
- MondayFriday 8:00am 4:30pm schedule with weekend and holiday rotations required

Experience
- 35 years experience required
- Must successfully complete a nationally recognized Advanced Practice Nurse certification examination or have current national certification; must maintain certification on an ongoing basis.
- Must be eligible for prescriptive authority under the criteria outlined by the Indiana State Board of Nursing; must maintain eligibility for prescriptive authority on an ongoing basis.
