Job Description

Overview:


Valid Ohio drivers license and proof of auto insurance is required by hospital policy. Must pass a motor vehicle background inspection insurance eligibility driving qualifications and training set forth by Nationwide Childrens Hospital and must maintain qualification of insurance guidelines.


Job Description Summary:


Assists patients with terminal or lifelimiting conditions and their families with pain symptom and plan of care management. Practices professional nursing and promotes patient advocacy in accordance with ANA Code of Ethics and maintains a current Ohio Nursing License.


Job Description:


Essential Functions:

  • Performs homebased care with familycentered approach.
  • Assesses individualized patient needs. Communicates patient care needs priorities updates and changes in condition per Board of Nursing Medicare/Medicaid Guidelines Joint Commission and the policies of Nationwide Childrens Hospital and Nationwide Childrens Hospital Home Health and Hospice.
  • Acts as the oncall nurse for a portion of each month divided among the nursing team.
  • Practices with a competent and compassionate approach to supporting patients and families facing terminal and lifelimiting conditions.
  • Actively participates in the interdisciplinary team and maintains their primary patients plan of care.


Education Requirement:

BSN required.


Licensure Requirement:

RN required.

Skills:

Must have excellent critical thinking skills good communication and customer service skills and the ability to organize and manage time well.

Ability to work independently as well as within an interdisciplinary team.

Necessary skills include but are not limited to pediatric assessment infusion and central line care injections wound care and patient education.

Must be comfortable with endoflife discussions and patient experiences.


Experience:

One year of pediatric experience required.

One year of hospice or palliative care experience preferred.
