Practice Manager St. Vincent de Paul Cincinnati (SVDP) has served residents of Greater Cincinnati for over 150 years with basic needs such as food medicine and homelessness prevention through its outreach center in the West End and 53 parishbased volunteer Conferences. SVDP provides holistic direct assistance and systemic change services while treating neighbors in need with dignity and compassion. Scope of Position: The Practice Manager is a nonclinical position that is essential to the mission of providing free prescription medication to lowincome neighbors in need. This position assesses and determines eligibility for patients certifying into the Charitable Pharmacy program as well as leading a team of staff and volunteer Patient Advocates. Functions also include scheduling eligibility review social determinants of health screenings documentation for audit preparedness training and oversight of volunteers and pursuing quality improvement initiatives in collaboration with leadership. Job Responsibilities: - Certify patients for Charitable Pharmacy services. Schedule certification appointments obtain complete information for audit readiness connect patients to necessary resources and advocate to fulfill patientspecific needs and perform data entry of all patient information in various databases.
- Supervise Patient Advocacy department. Manage multiple patient advocates recruit and train volunteers develop efficiencies to improve patient care.
- Coordinate multisite staffing schedules and supplies. Ensure staff/volunteer schedules provide adequate coverage and supplies to support patient appointments across multiple locations of the Charitable Pharmacy.
- Lead quality improvement initiatives. Collaborate with staff to improve effectiveness through initiatives that impact care coordination data tracking and reporting and overcoming barriers to patient health.
- Other duties as assigned.
Qualifications:Masters in health administration public health social work or related field. Strong organizational attention to detail interpersonal written and oral communication. Experience in healthcare databases and data management. Ability to work with people of diverse backgrounds in a faithbased setting. Certified pharmacy technician preferred. Work Environment:Staff member will primarily work in an office environment adequately lighted heated and ventilated at multiple locations within Greater Cincinnati.
