Job Description
Physical Therapist (PT)
A Physical Therapist is needed in Port Charlotte Florida.
Port Charlotte is a growing community along Floridas Gulf Coast offering affordable living waterfront activities and a relaxed familyfriendly atmosphere. This role allows you to make a meaningful impact while working in a supportive home health environment dedicated to patient care and professional growth.
Salary and Benefits
- Paypervisit model: $60 $67 per point (based on experience; $67 for 20 years)
- Mileage reimbursement: $0.585 per mile

Responsibilities
- Provide skilled physical therapy services in a home health setting
- Conduct comprehensive patient assessments including OASIS and ongoing evaluations
- Create and implement individualized treatment plans in collaboration with interdisciplinary care teams
- Deliver oneonone patient care focused on improving mobility and independence
- EMR system: Kinnser/WellSky preferred but not required
- Participate in weekend holiday or oncall rotations as needed
- Patient assignments are geographically grouped to reduce travel
- Supportive management team with opportunities for career advancement
- PRN rates available for productivity beyond base expectations
Productivity Expectations
- 30 points per week / 60 points biweekly
- Start of Care OASIS: 2.5 points
- Resumption of Care: 1.75 points
- Recertification: 1.75 points
- Evaluation visit: 1.25 points
- Discharge OASIS: 1.5 points
- Routine visit: 1.0 point
- Precepting: 0.5 points visit fee
Requirements
- Active Florida PT license
- At least 1 year of home health experience preferred
- Current CPR/BLS certification
- MondayFriday availability (most visits between 8 AM 5 PM)
- Ability to cover Port Charlotte and surrounding areas
