Job Description
Nurse Practitioner (NP)
A Nurse Practitioner is needed in Bridgeport Connecticut.
Bridgeport is a historic coastal city offering a blend of communityfocused living arts and culture and easy access to major metropolitan areas. This opportunity is with a missiondriven healthcare organization that provides highquality valuebased care to the Medicare population.
Salary and Benefits
- Base salary: $105000 $135000 (depending on experience and location)
- Incentive bonus potential: $15000 $25000 annually (based on quality metrics)
- Physician partnership track available
- 6 weeks of total paid time off (includes PTO holidays and CME time)
- CME allowance: $5000 annually

Responsibilities
- Fulltime MondayFriday schedule 8:00 AM 5:00 PM)
- Patient volume: 1316 patients per day
- Visit structure:
- Regular visits 20 minutes
- Full diagnostic/new patient visits 40 minutes
- Work within a valuebased healthcare model
- Supported by a care team including a scribe medical assistant and social worker
- Onsite labs with phlebotomy services and access to RubiconMD for specialty consultations
- EMR systems: Canopy and Greenway
- Participate in community wellness activities (exercise programs yoga dance health events)
- Average of 35 providers per clinic (mix of NPs and Primary Care Physicians)
Requirements
- Master of Science in Nursing Doctor of Nursing Practice or completion of an accredited Nurse Practitioner certificate program
- Active and unrestricted Nurse Practitioner license in Connecticut
- Active DEA license
- National board certification in one of the following:
- Family Nurse Practitioner (FNP)
- AdultGerontology Primary Care Nurse Practitioner (AGPCNP)
- Adult Nurse Practitioner (ANP)
- Gerontological Nurse Practitioner (GNP)
- Familiarity with HCC documentation ICD10 coding and Health Risk Assessments (preferred)
