Job Summary
We are seeking a dynamic organized and flexible selfstarter to fill the role of Community Intervention Specialist (CIS) for a shared position across two programs: the Community Outreach Team (COT) and Healing Hurt People (HHP). This position will be split with 60 of the time dedicated to HHP and 40 to COT. The CIS will provide traumaspecific services in both communitybased and hospital settings working with violently injured individuals and their families in various environments including schools homes and communitybased locations. This role is based out of the COT/HHP main office in Center City and St. Christophers Hospital for Children and it will involve travel to a range of community and partner organizations.
COT Overview:
The Community Outreach Team (COT) at the Center for Nonviolence and Social Justice provides assertive outreach engagement and therapeutic interventions to young people receiving services at local youthfocused organizations. The CIS will be responsible for providing acute intervention outreach traumafocused counseling group facilitation and case management for children and adolescents (ages 1418 and young adults (up to age 24 who are victims of or witnesses to interpersonal violence.
HHP Overview:
Healing Hurt People (HHP) is a traumainformed program dedicated to addressing the longterm consequences of trauma and promoting emotional and physical healing among violently injured young people and witnesses of violence. The CIS will provide acute intervention outreach traumafocused counseling and case management for children adolescents and adults (ages 5 to 35 focusing on community hospital office and homebased services.
Essential Functions
- This shared role requires flexibility excellent time management skills and the ability to balance responsibilities between both programs. The CIS will play a key role in providing lifechanging services to vulnerable individuals in the Philadelphia community supporting their healing journey and promoting traumainformed care within local organizations.
- Community Intervention 60 HHP / 40 COT)
- Provide traumafocused counseling and advocacy for individuals aged 5 to 35 both in the hospital and community (HHP) and for adolescents (ages 1418 and young adults (up to age 24 in the community (COT).
- Conduct intake evaluations for new clients and maintain a caseload of 10 to 20 clients adjusting based on client needs and productivity requirements 12 to 15 billable weekly hours)
- Offer individual family collateral and group therapy sessions using evidencebased modalities such as Motivational Interviewing (MI) TraumaFocused Cognitive Behavioral Therapy (TFCBT) Cognitive Processing Therapy (CPT) and other traumafocused interventions.
- Provide referrals and intensive case management to connect clients with communitybased resources including employment school and social services.
- Cofacilitate 25 weekly CYPHER groups a traumainformed group intervention both in hospital and community settings.
- Collaborate with community partners and schools to implement traumainformed initiatives and staff education including quarterly workshops and trainings.
- Maintain accurate and timely documentation for all client interactions including treatment plans progress notes and billing records in compliance with PCCD CBH and state standards.
- Maintain intensive followup contact with clients families and service providers through home visits telephone calls and communitybased engagement.
- Provide individual family and group traumainformed counseling to clients in both community and hospital settings.
- Complete intake evaluations and maintain caseloads adjusting based on showrates and productivity needs.
- Engage with caregivers facilitate family therapy sessions and collaborate with key stakeholders to support client progress.
- Cofacilitate trauma recovery groups Community of Young People Healing Experiencing and Rebuilding (CYPHER) and provide educational initiatives for partner organizations. Maintain case records and ensure timely documentation in compliance with CBH PCCD and state regulatory standards.
- Participate in supervision case conferences and ongoing training to maintain high standards of service delivery. (Essential)
- Additional Responsibilities:
- Collaborate with both COT and HHP Program Directors on strategic initiatives community engagement and service planning.
- Assist with organizing and attending community resource fairs stakeholder events and trainings as part of broader outreach efforts.
- Other duties as assigned by supervisors from both COT and HHP.
Required Qualifications
- Minimum of Masters Degree in Social work Psychology Counseling or Family Therapy orthe equivalent combination of education and work experience.
- Licensed in Pennsylvania (LSW LCSW LPC or LMFT) or the ability to become licensed at the time of hire
- Ability to work evening hours at least three times per week and engage in community based work.
- Bilingual (English/Spanish) required.

Additional Information
This position is classified as Exempt grade I. Compensation for this grade ranges from$41120.00 to $61680 Per year . Please note that the offered rate for thisposition typically aligns with the minimum to midrange of this grade but it can vary based on thesuccessful candidates qualifications and experience department budget and an internal equity review.
