JOB SUMMARY:
Under the general supervision of the PTW Associate Vice President the Health Home Supervisor provides Clinical oversight for PTW Care Coordination and Outreach Services for Children and Adults. The HH Outreach Supervisor is responsible for assigning monitoring new outreach and enrollments to the program and ensuring that the clients are connected to the appropriate services. Quality Improvement functions include reviewing their teams caseload on a biweekly basis and ensuring that all program documentation is complete and submitted in a timely manner. The HH Outreach Supervisor is also responsible for reviewing the quality of documentation and ensuring it meets agency and programmatic standards. Weekly supervision is expected which focuses on clinical issues as well as chart reviews. Establishes and maintains systems to ensure that Team operations meet or exceed agency and regulatory guidelines and that documentation and billing conform to all applicable targets rules and regulations.

Provides Clinical Oversight to Team Members:
1. Oversees intake and screening activities of team to ensure that service recipients are appropriate for enrollment in the program.
2. Participate in community outreach and fairs to assist with the enrollment census of the program.
3. Supervises the assessment and reassessment activities of staff; ensuring that appropriate determinations and evaluations are made that Medicaid and demographic information is accurate.
4. Review referrals assigned to assess their eligibility for outreach.
5. Reviews new enrollments to assess whether or not appropriate service linkages are developed and maintained.
6. Supervises the development and implementation of comprehensive Care Plans and Assessments are completed and ensures that they are maintained in a timely accurate and complete manner.
7. Supervises Care Coordination staff in the provision of crisis intervention services reviews crisis management plan and revisions of Care Plans as appropriate.
8. Assures that quality services identified in the Care Plan are delivered in a timely manner and that the client is satisfied with the services provided.
9. Supervises the counseling and exit planning provided by Care Coordinators and ensures that appropriate discharge planning is provided.
10. Immediately reports serious incidents allegations of abuse (including the reporting of child abuse or neglect) or sensitive situations and completes incident reports in accordance with applicable agency and regulatory policies.
11. Regularly provides individual and group supervision treatment team meetings case conferences inservice training and other staff development activities.
Directs and Supervises Personnel:
1. Responsible for performance management of staff provides regular feedback to supervisees both orally and in writing invoking the disciplinary process as needed.
2. Monitors employee morale and the climate of the work environment to stop or eliminate sexual harassment or any form of employee harassment or employee discrimination. Responsible for taking complaints of sexual harassment informing the Human Resources Department of the complaint and investigating the complaint (as directed).
3. Monitors employee accrual totals approves or recommends approval of employee requests for time off and notifies Human Resources when an employee should be placed on or taken off Family and Medical Leave (FMLA). Maintains contact with staff while out on FMLA.
4. Provides oversight in the area of administrative record keeping billing and maintenance of office equipment.
Provides Quality Assurance Oversight:
1. Ensures internal program compliance with federal state city and agency regulatory requirements. Monitors the quality of the services provided to clients.
2. Responsible for the development of a program mission statement and program goals that support the division and agency mission statements and goals.
3. Coordinates with the Quality Assurance/Corporate Compliance Department and the appropriate corporate administrative departments in preparations for audits.
4. Develops as requested initial plans of corrective action incident reports and program evaluation reports and addresses areas that need improvement.
5. Provides oversight into the submission of timely accurate and complete statistical data.

6. Good knowledge of the policies and organization of the regulatory agency and applicable legislation regulations and rules.
7. Good knowledge of the principles and practices of supervision.
8. Basic knowledge of best practices evidencebased treatment and general treatment techniques.
9. Ability to plan and organize treatment services and to provide supervision and leadership to a culturally diverse group of staff.
10. Ability to assist in the preparation of written proposals for new or expanded treatment services.
11. Ability to identify from the record and from observation deficiencies in individual treatment plans.
12. Excellent oral and written communication skills.
13. Ability to efficiently and effectively manage resources including money material time and people
QUALIFICATIONS AND EXPERIENCE:
Licensed by the NYS Education Department as a mental health practitioner: Social Worker Marriage and Family Therapist Creative Arts Therapist or Psychologist. At least 1 year of experience in a supervisory capacity in a behavioral health clinic or care management setting.
OR
Masters degree in a professional mental health discipline (see above) and 3 years experience supervising clinicians and/or case managers providing direct services to individuals with serious mental illness (SMI) and/or serious substance use disorder (SUD).
