Job Description:The Palliative Care Coordinator will oversee clinical services that reflect the organizations philosophy and mission; and the programs goals of quality patient care and judicious use of resources. The Palliative Care Coordinator will coordinate resources that will focus on improving life and providing comfort to people of all ages with serious chronic and lifethreatening illnesses.Qualifications:- State of Texas RN license required
- 3 years experience managing a palliative care and/or hospice program required
- 3 years experience in management required
- Must have experience in keeping the agency updated with changes in rules and regulations governing the delivery of palliative care services
- Must have experience organizing and directing the agencys ongoing function of the palliative care program.
- Must have experience with making palliative care and/or hospice presentations
- Masters or Bachelors degree required or five years equivalent experience required
- Able to multitask
- Effective written and verbal communication
- Clean back ground and drug screen
Responsibilities:- Responsible for organizing and directing the ongoing function of the palliative care program
- Assures that all programs and activities meet local state and federal requirements
- Monitor Palliative Care Admissions staffing and conversions. Will report this information on EOD reports
- Market HarborCare and oversee HarborCare handouts for marketing purposes
