Bridges Liaison
Pay: $20.19/hour
This is a communitybased role daily travel required.
Bridges Liaisons with National Youth Advocate Program work in the community as part of an integral service team. In this role you will provide support and guidance to young adults eligible for the state administered Bridges Program as they transition out of foster you will assist with transportation of youth while developing a trusting relationship.
Under the general supervision of the Bridges Supervisor or designee you will work closely with youth family foster parents and community partners providing advocacy and support toward the ultimate goal of living safely and successfully in the community.

Responsibilities
Facilitates eligibility and enrollment into the Bridges Program.
Completes home visits and assessments.
Ensures Bridges eligibility documents are maintained in SACWIS for IVE payment.
Completes Bridges assessments and performs onsite evaluations.
Assist with the completion of tier identification and Bridges assessment updates.
Completes goal setting through the development of the Bridges plan.
Identify initiate and coordinate housing referrals and care management needs.
Appears in Court with Bridges participants.
Reviews Bridges plans to maintain Bridges eligibility.
Teach independent living skills.
Supports postsecondary school enrollment vocational program enrollment or securing employment.
Connects physical and mental health care and community resources for selfsufficiency.
Provide interventions as needed withclient(s)while being transported to and from various appointments and visits throughout the community.
Participate in the development of the youth treatment/service plans asneeded.
Interact with the youth to develop a trusting supportive relationship while assistingachieving identifiedgoals.
Provide PSR and/or TBS Services(Training Provided).
Ability to engage clients; document conversations and outcomes and submit paperwork for billing.
Minimum Qualifications
Bachelors Degree in social work human services behavioral health or related field.
Experience working with families in the child welfare system.
Daily travel required with the willingness to meet clients within 1.5 hour radius on a flexible schedule or during nontraditional hours if needed.
Must be comfortable providing services in the community and in client homes.
Must be willing to perform other duties assigned.
Must be at least 21 years of age.
