Schedule Opening:
- parttime 20 hours a week
- more hours available if willing to cover Eau Claire locations
- flexible schedule including days and hours
- paid drive time and mileage reimbursement
- supporting our locations in Bloomer and Chippewa Falls onsite and virtually

Responsibilities:
- plan develop organize implement evaluate and direct physical therapy as well as its programs and activities in accordance with current rules regulations and guidelines that govern a skilled nursing facility
- participate in discharge planning development and implementation of patient care plans patient assessments etc. as necessary
- maintain treatment records patient files and progress notes as required
- participate in the development and implementation of patient assessments and care plans including quarterly reviews
Required Qualifications:
- current license as an physical therapist in the State of Wisconsin
- ability to support our locations both onsite and virtually
