Job Description

Premise offers a wide range of dynamic purposedriven career opportunities. We are currently looking for a fulltime Physical Therapist Supervisor to join our team in Lafayette IN!



Essential Functions:

Works in collaboration with other Health Center staff members including doctors NPs PAs nurses dietitians wellness coaches as well as community providers to promote health and wellness to onsite employees and their dependents.
Oversees staff which may include PTs PTAs Athletic Trainers Exercise Physiologists administrative and technical staff (staff may be exempt or nonexempt).
Responsible for staffing onboarding performance coaching and staff development process for applicable team members.
Manages daily operations workflow work schedules timecard monitoring etc. to ensure efficient and effective performance.
Interfaces with provider team to develop a plan of care for members.
Maintains and provides data to support the success of the Physical Therapy team to include providing monthly reporting.
Identifies process improvement opportunities and presents resolutions and recommendations to Health Center Manager or the Director of Site Operations.
Understands and complies with all regulatory procedural policy and licensing requirements.
Completes incident reports as needed and handles the resolution of complaints.
Communicates and works collaboratively with the Health Center Manager or Director of Site Operations at the site to manage internal site issues
May require other duties as assigned




Job Requirements:

All state licenses/certifications to practice
Current board certificate (if applicable)
Degree from accredited Physical/Occupational Therapy School
CPR (BLS for the Healthcare Provider/Professional Rescuer the copy must include the front and back of card as applicable) Certification must include hands on evaluation of skills by the trainer/instructor. Total online courses will not be accepted. Course must also meet the American Heart Association (AHA) standards but need not be an AHA certificate.
At least three years experience as a Physical Therapist in an outpatient physical therapy clinic or other ambulatory care clinic. Outpatient orthopedic experience is required.


Preferred Experience:

Knowledge of Internet software and Word Processing software.
Knowledge and experience with Electronic Medical Records preferred.
Knowledge of workplace health and safety concepts and OSHA regulations in occupational health settings preferred.
