Job Description
Located in Fremont Ohio ProMedica Memorial Hospital has served the citizens of Sandusky County and the adjacent communities since 1918. In fact our hospital was built with funds raised by members of our community on land donated by the family of former United States President Rutherford B. Hayes.
POSITION SUMMARY
Perform clinical pharmacy duties under the direction of the Lead Clinical Pharmacist (if applicable). Clinical specialists support the development implementation and maintenance of systems that promote desirable patient outcomes through the use of safe appropriate costeffective medication therapies.
Requirements
- Education: PharmD degree or BSPharm with experience since 2006 plus PGY1 Pharmacy Residency
- Clinical Skills: Patient counseling patient profile review pharmacokinetic evaluation superior time and project management preceptorship antimicrobial basic skills evidencebased literature review
- Operational Skills: narrow service area expertise for drug information needed for preparation administration and delivery of medications pharmacokinetic evaluation necessary for narrow service area superior time and project management evidencebased literature review
- Years of Experience: Completion of PGY1 or 3 years full time inpatient pharmacy practice.
- License: Must be a registered pharmacist licensed in good standing in Michigan and Ohio. Single facilitybased pharmacists must retain state licensure per site location.
- Certification: BLS ACLS and/or PALS Board Certification within 2 years of hire
PREFERRED QUALIFICATIONS
Years of Experience: Completion of PGY 2 or 4 years full time inpatient pharmacy practice.
Certification: Board Certification
WORKING CONDITIONS
- Personal Protective Equipment:
- Physical Demands: Must be able to lift light materials. Nonmaterial handling of continuous sitting and reaching; frequent standing; occasional squatting kneeling walking and stairs. Must be able to move about and between workstations.
