Senior Home Health Physical Therapist $95000$110000

Were seeking an experienced compassionate physical therapist to deliver exceptional rehabilitative care to patients in their homes. Key Responsibilities:
- Conduct comprehensive physical therapy evaluations and assessments in patients homes
- Develop and implement individualized treatment plans aligned with patient goals
- Design and teach therapeutic exercise programs for optimal patient outcomes
- Provide expert education to patients and caregivers on injury prevention and wellness
- Document patient progress and maintain detailed clinical records
- Collaborate with interdisciplinary healthcare teams to ensure comprehensive care
- Adapt treatment approaches to suit various home environments and patient needs
Required Qualifications:
- Masters degree in Physical Therapy required (DPT preferred)
- Current state Physical Therapy license
- Minimum 1 year of home health experience
- Valid drivers license and reliable transportation
- Current CPR certification
- Strong communication and interpersonal skills
- Proficiency in electronic medical records systems
