Job Description

Bilingual Health Services Care Manager (Spanish)

Are you looking for a meaningful job that can truly make a difference in someones life then come join the Mystic Valley Team! The Bilingual Health Services Care Manager/Case Manager is a communitybased position where you can build relationships with older adults by providing case management.


Schedule: This is a Local Union 509 fulltime nonexempt 35 hour/week) position. We offer 4day work week and hybrid work schedule (once training is complete. No nights or weekends.


Compensation: Starting annual salary is $49874.92. Being bilingual in Spanish is required for this role we offer an additional 6 bilingual designation. Enjoy benefits that start as of day 1 of employment and our generous paid time off package.

Job Activities

Job activities of the position are below. Additional duties may be assigned as required.

  • Conduct initial and ongoing assessment of consumers health and functional status.
  • Conduct visits as required in various settings including consumers homes community settings and/or medical facilities.
  • Participate in development of consumers care plans including determining appropriateness and need for inhome/communitybased services.
  • Participate as part of consumers primary care team including attending team meetings trainings and joint home visits.
  • Arrange coordinate and oversee inhome and communitybased services for consumers.
  • Monitor consumers health status in conjunction with care team to assess for effectiveness of current services need for additional services and/or need for longterm placement.
  • Assist consumers with MassHealth redeterminations and other benefits paperwork as requested and appropriate.
  • Maintain consumer records in accordance with standards.
  • Compile and submit required reports.

Qualifications:

  • A Bachelors degree in Social Work or a related field.
  • Fluent in English and Spanish
  • Social Work license preferred.
  • Knowledge of and experience working with older adults in a communitybased setting preferred.
  • Strong interviewing and assessment skills.
  • Strong interpersonal and organizational skills.
  • Excellent written and verbal communication skills.
  • Computer skills: Proficient with MS Office Suite and entering narrative and other data into a database. Ability to use multiple databases. Ability to use the Internet to conduct information searches
  • Private transportation.
