Position Overview
As a Home Health Physical Therapist youll deliver personalized rehabilitation services to patients recovering from injuries surgeries and various medical conditions. This role offers flexible scheduling competitive compensation and the opportunity to build lasting therapeutic relationships.
Essential Responsibilities
- Perform comprehensive physical assessments and evaluate patients functional capabilities
- Develop and implement individualized treatment plans aligned with patient goals

- Document patient progress and maintain detailed electronic health records

Required Qualifications
- Masters degree in Physical Therapy from an accredited program
- Current state Physical Therapy license in good standing
- Minimum 1 year of home health experience
- Valid drivers license and reliable transportation
- Current CPR certification
- Strong communication and interpersonal skills
- Proficiency in electronic documentation systems
