LOCATION: Venice CA

STATUS: FullTime 40 hours/week Exempt

ABOUT THE ROLE:


A member of the Access Center team the Access Center Case Management Supervisor is responsible for the oversight of daily operations of case management activities including supervising the Member Services team which consists of Case Managers Problem Solving Specialists and Housing Navigators. They will ensure that all case management functions are performed accurately and efficiently and in compliance with all applicable state and federal regulatory requirements. This position requires extensive coordination within multiple internal SPY Programs and external systems that include relationships with the Housing Authority DCFS DMH LAHSA St. Josephs Center community partners and resources landlords and property owners and managers.


WHAT YOULL DO:


  • Develop implement and supervise case management workflows in compliance with all applicable state and federal regulatory requirements
  • Support the daytoday operations and programing of the Access Center including site monitoring
  • Supervise direct service staff in providing case management housing navigation and problem solving
  • Work closely with the Access Center Manager to administer problem solving funds and financial assistance
  • Review case management notes and service plans to ensure that all members served are receiving high quality care
  • Monitor and provide continuous quality improvement of member charts data collection and analysis monthly reports and program administration duties in a timely fashion
  • Ensure that program outcomes are being met and that performance and quality improvement strategies are being embedded in the work
  • Developing a high performing department culture and staff. This includes setting the standard for staff/peers and motivating employees to maximize organizational goals and objectives.
  • Setting goals and planning assigning and directing work consistent with said goals. This includes responding to employees needs ensuring they have the necessary resources to do their work.
  • Appraising performance rewarding and disciplining employees addressing complaints and resolving issues. This includes providing regular and effective feedback to employees and completing timely and objective performance reviews.
  • Provide individual and group supervision to staff
  • Facilitate case assignment case conferencing and other relevant care coordination or treatment planning meetings
  • Act as a preceptor for MSW/MFT interns
  • Provide mediation and crisis intervention to youth as needed
  • Advocate by attending external community meetings with government agencies and community groups
  • Work within established legal and ethical guidelines
  • Ability to work nights/weekends as needed
  • Other duties as assigned to support the mission of the organization.


Requirements

BACKGROUND YOULL NEED:

  • Bachelors Degree in Social Work Psychology Sociology or related field OR comparable experience is preferred.
  • 34 years of case management or related experience working with unhoused and atrisk TAY is required.
  • 12 years of experience working in a dropin center access center or access point is preferred.
  • 12 years of experience supervising direct service staff is required.
  • Knowledge and experience working with cooccurring populations (chronic health conditions mental health and substance abuse) preferred
  • Knowledge and experience working with LAbased housing resources partners and programs.
  • Best practice models including: Housing First Positive Youth Development Authentic Youth Engagement Motivational Interviewing Harm Reduction TraumaInformed Care Critical Time Intervention.
  • SelfStarter skill set requiring the ability to develop relationships build rapport and trust with members community partners volunteers and staff.
  • Strong verbal and written communication skills including organizational detail and interpersonal skills.
  • Ability to uphold Access Center policy and procedure with members staff volunteers and community partners
  • Ability to work effectively with people of diverse races ethnicities sexual orientations and gender identities in a multicultural environment.

  • WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:
  • Prolonged periods of sitting at a desk and working on a computer.
  • Changes in the environment such as the office or outdoors.
  • Physical effort/lifting such as sedentary up to 1520 pounds at times.

SPECIAL CONDITIONS:

Mandated Reporting Requirements: The person holding this position is considered a mandated reporter under the California Child Abuse and Neglect Reporting Act and is required to comply with the requirements set forth in CSU Executive Order 1083 Revised July 21 2017 as a condition of employment.

Driver Positions:

Applicants are required to provide a copy of their driving record from the Department of Motor Vehicles along with a copy of their valid drivers license and proof of insurance. Job offers are contingent upon a satisfactory driving record. Examples of an unsatisfactory driving record include:

  • Suspended or revoked license
  • Three or more moving violations in the past 36 months
  • Any violations including alcohol controlled substances within the past 24 months
  • Reckless driving including hit and runs within the past 24 months
  • At fault accidents resulting in fatality or serious injury within the past 5 years
