ACO Triage Nurse
Full Time Position
Remote Position
Work schedule includes: shifts including weekends and holidays EST zone
Essential Duties and Responsibilities
- Participate in the AfterHours OnCall Team rotation
- Answer all incoming calls immediately and give correct advice
- Provide PostAcute Care Outreach
- Assist Hospitalists Patient Outreach
- Facilitate Prescription Refills
- Assist RN Care Management Teams with patient assessments and/or monthly followup calls when needed
- Monitor Patients enrolled in the Patient Monitoring Program
Required Knowledge and Experience
- Registered Nurse licensed in FL
- License must always be kept current
- 5 Years of clinical experience with 1 year(s) of case management experience preferred
- ER/ICU/Triage experience is preferred.
- Telephonic triage experience
