ABOUT THIS POSITION:
The Department of Family Services (DFS) Adult and Aging Division welcomes applicants interested in the Public Health Nurse II position. The position assesses functional eligibility for Medicaidfunded LongTerm Services and Supports (LTSS) including CCC Nursing Facility PACE Adult Day Health and other services. Works as a certified LTSS Screener on multidisciplinary screening teams conducting home visits and making an initial determination as to clients health and safety needs and their current level of care state and federal guidelines. Implements public health nursing activities providing services and intervention to include outreach case findings health promotion education care coordination consultation and disease prevention. This position also provides case management services for older adults and adults with disabilities including comprehensive assessments using Virginias Uniform Assessment Instrument (UAI) care plan development and coordination and crisis intervention. Uses a personcentered practice cultural agility engaging communication techniques and critical thinking skills. This position attends various agency trainings required for maintaining core nursing skills. This position performs other duties in support of the DFS mission goals and objectives.- Works as a certified LongTerm Services and Supports (LTSS) Screener with a multidisciplinary screening team.
- Consults on screenings and all other Adult and Aging inquiries where applicable.
- Conducts home visits and make an initial determination as to the clients health and safety needs and their current level of care per Department of Medical Assistance Services (DMAS) guidelines.
- Conducts field visits to a diverse population including at risk medically fragile disabled seniors adults and children.
- Uses a personcentered practice cultural agility engaging communication techniques and critical thinking skills.
- Works both independently and collaboratively respecting opinions of others and considering what clients choice is regarding their plan of care.
- Makes appropriate referrals and recommendations.
- Implements public health nursing activities providing services and intervention to include outreach case findings health promotion education care coordination and disease prevention.
- Attends required meetings and participates in training.
- Assesses risks posed to clients.
- Interprets state and local policy; mentors and trains new staff; conducts trainings for staff and provides educational presentations to community groups.
- Attends and participates in local IDT meetings.
- This position requires assessment care planning and coordination skills; the ability to manage competing priorities and to respond effectively to crisis situations.
The primary purpose of the Adult and Aging Division is to ensure resources advocacy safety and wellbeing of older adults adults with disabilities and caregivers are met. To ensure that the division can carry out its purpose and meet the agencys responsibilities related to advocacy protection and wellbeing the Adult and Aging division director may move or reassign this positions related work duties and responsibilities within the job class. Illustrative Duties
Complies with professional standards policies regulations and procedures based on federal and state laws and regulations and county community health policies and procedures;
Gathers health and social histories from clients using a standard protocol;
Performs total client assessment by aggregating analyzing and interpreting health assessment findings to determine health care needs;
Acts as case manager to establish appropriate care plan for individual clients and client groups;
Documents health care service utilization client outcomes and information regarding referrals and followup of suspected deficiencies (e.g. scoliosis vision or hearing defects);
Aggregates and reports statistics across cases according to established policies protocols and procedures;
Provides input into client education needs and learning objectives;
Develops teaching plans and contributes to the development and delivery of health education classes to community groups;
Responds to or seeks information on responses to complex questions from clients the public and other health care providers including questions about community resources;
Plans implements and evaluates community or school educations programs (e.g. health promotion safety universal precautions HIV/AIDS education);
Participates in county nursing groups task forces committees or subcommittees and in interintradepartmental nursing committees and subcommittees;
Serves as a liaison between the countys health departments and other entities within and outside of the county (e.g. school system agencies with other counties) to ensure effective delivery of public health services;
Provides screening of individuals in highrisk groups via community health fairs and/or senior site programs and intervenes on behalf of individuals in need;
Participates on inter/intraagency case management teams providing consultation and nursing services;
Interprets guidelines and assists the community and schools in the implementation of federal state and county regulations including Part H child abuse reporting communicable disease control and immunizations;
Participates in inter/intradepartmental nursing committees and subcommittees;
Attends disciplinespecific training and workshops;
Orients and coaches new employees;
Assists in training new employees by teaching documentation and demonstrating procedures according to established policies protocols procedures and guidance.Required Knowledge Skills and Abilities
Knowledge of nursing and public health theories principles practices methods processes and procedures required to provide public health nursing;
Knowledge of human growth and development anatomy and physiology pharmacology (e.g. side effects contraindications) preventive medicine epidemiology and nutrition;
Knowledge of current practices in public health nursing service delivery;
Knowledge of available community resources for referral or the ability to rapidly acquire such knowledge;
Knowledge of documentation nursing diagnosis and quality assurance;
Knowledge of professional standards of nursing practice;
Knowledge of applicable federal state and county laws and regulations;
Knowledge of county Health Department policies and procedures;
Ability to develop and maintain effective working relationships with others including those from varied backgrounds and different situations in individual and teambased settings;
Ability to handle contentious interactions especially with those who may appear difficulthostile or distressed;
Ability to identify problems and make sound wellinformed and objective decisions or judgments relating to client health care;
Ability to assess the quality appropriateness implications and impact of decisions or judgments made by others and identify courses of corrective action where appropriate;
Ability to communicate effectively both orally and in writing with individual or small and large group interactions;
Ability to provide health training to clients and families with complex multiple health needs using knowledge of adult learning concepts;
Ability to accurately maintain client medical records and information (e.g. filing retrieval recording and archiving) and to protect the privacy of clients served;
Ability to use technology to document file retrieve make adjustments to medical records or information and communicate or share public health information in the appropriate format;
Ability to assign paraprofessionals volunteers and students according to ability level and set work priorities according to clinic and/or caseload demands.Employment Standards
MINIMUM QUALIFICATIONS:
Graduation from a college or university accredited by the National League for Nursing or the Commission on Collegiate Nursing Education with a bachelors degree in Nursing; plus one year postlicensure nursing experience.
CERTIFICATES AND LICENSES REQUIRED:- Possession of current license or a multistate licensure privilege to practice as a Registered Nurse in the Commonwealth of Virginia.
- Valid Drivers License.
- AEDRequired within 60 days.
- CPR Required within 60 days.
NECESSARY SPECIAL REQUIREMENTS:
The appointee to this position will be required to complete a criminal background check a sanction screening a check of the Child Protective Services Registry and a driving record check to the satisfaction of the employer. Tuberculosis test.
This position is considered essential personnel and will be required to report to work during times designated countywide as approved for unscheduled leave regardless of the emergency situation (i.e. weather transportation other disaster).
PREFERRED QUALIFICATIONS:- Certified as a LongTerm Services and Supports (LTSS) Screener.
- Working knowledge of communitybased and longterm care services.
- Demonstrates skills necessary for doing basic and advanced health care assessments in the home or community setting.
- Experience working with a diverse population including at risk medically fragile and/or disabled seniors adults and children.
- Familiar with Department of Medical Assistance (DMAS) regulations and Virginia Department of Health (VDH) code regarding LTSS.
- Demonstrates ability to assess the home to identify need for adaptive changes and intervenes to facilitate changes in client care and/or the environment.
- Proficient computer and software skills to include Microsoft Office Suite virtual meetings electronic signing/exchanging of required forms and electronic medical records system.
PHYSICAL REQUIREMENTS:
Sufficiently mobile to do home visits and able to communicate with clients. Ability to use automated technology. Ability to communicate clearly and concisely both orally and in writing. All duties performed with or without reasonable accommodations.SELECTION PROCEDURE:
