Job Description

Classification Title:

Clinical Programs Coord III

Classification Minimum Requirements:

Bachelors degree in an appropriate area and four years of relevant experience.

Job Description:
  • Assigns newly enrolled clients to case managers in a manner that will balancethe workload of the care services staff members while also ensuring that all clients receive the level of support and attention they need.
  • Monitors clientrelated record keeping and case notes for accuracycompleteness and compliance with relevant patient/client privacy standards.
  • Establishes and maintains productive collaboration and strategic affiliations withproviders of health and social services in Jacksonville TGA to ensure that clientshave effective access to health care housing mental health services substanceabuse treatment and other relevant services.
  • Takes steps to ensure compliance with requirements imposed by funding sourcesincluding accurate record keeping and timely reporting.
  • Participates in activities to educate the community especially local health andsocial services organizations and including health fairs and other events onHIV/AIDS and about the role and services of UF CARES.
  • Oversees and/or directs employee management activities within UF CARESMedical Case Management program staff including interviewing hiring trainingcounseling and disciplinary actions. Routinely conducts staff meetings to discussoperations staff issues and customer satisfaction; reviews new policies and
    procedures and shares general information.
  • Participates in various organizational management meetings and committees:Ryan White Committees Clinical Effectiveness Group (CQI) HIV classesLeadership Development Institute Directors meetings and ensuring the servicesfor PAC Waiver.
Expected Salary:

$59000 to $70000 annually depending on education and experience.

Required Qualifications:

Bachelors degree in an appropriate area and four years of relevant experience.

Preferred:
  • Masters degree in a relevant discipline such as social work nursing public health or public administration and four years of appropriate experience.
  • Bachelors degree in an appropriate area of specialization and six years of appropriate experience.
  • HIV/AIDS and substance abuse prevention or service coordination experience with the target population preferred.

This is a timelimited position. Time limited positions are based on circumstances such as funding sources which control the length of time for which the position is available.
