POSITION SUMMARY

The Community Living Professional Manager will support the CHS mission by participating as an active member of the Community Living management team and providing leadership supervision training and overall coordination of activities at various assigned locations to successfully support Direct Support Professionals and the Individuals we serve. The Community Living department is a 365 day/24hour service line.

ESSENTIAL DUTIES AND RESPONSIBILITIES
As the Community Living Professional Manager you will:

  • Affirm the success of team members by scheduling and training new talent including Individual Support Plan (ISP)training: providing constructive and timely feedback through performance evaluations or during everyday interactionsto assigned employees; and handling discipline and termination of employees in accordance with company policy.
  • Project a positive image of the Center for Human Services by interacting with staff members external agencies parents
    or guardians and providers in a courteous and professional manner.
  • Create a culture of communication by attending and leading administrative staff meetings; informing the management
    team of developments or concerns; working with staff members to address issues or concerns through critical thinking
    and problemsolving; participating in ISP meetings or other team meetings regarding Individuals served; and
    coordinating with Individuals families support coordinators other service providers and the Division of Mental Health
    (DMH) and other state and local governmental departments and regulatory agencies.
  • Maintain integrity of records by ensuring Medication Administration Record (MARs) are accurate through oversight
    and supervision; writing implementation strategies related to ISPs for individuals served; ensuring discharge summaries
    are completed for all Individuals served by the Community Living Program; performing quality enhancement
    monitoring; and reviewing and approving documentation; supporting Individual financial health through oversight and
    assistance with personal funds which may involve tracking systems during service delivery.
  • Support the health and welfare of the Individuals served by administering prescription and PRN medications as
    prescribed by physician; collaborating with the Community Registered Nurse on the individuals medical needs; ensuring
    physical examinations are completed annually or as needed.
  • Ensuring sufficient workforce coverage by completing shifts to provide direct support when coverage cannot be
    satisfied by other means; participating in a tiered oncall rotation being available by phone 24 hours a day to provide
    immediate assistance responding physically within 30 minutes in areas where remote supports are provided/ 1 hour in
    areas where remote supports are not provided; and maintaining unimpaired cognitive clarity during the oncall period.

QUALIFICATIONS


REQUIRED EDUCATION and EXPERIENCE

  • Bachelors degree (BA or BS) from an accredited university required (Direct experience or a combination of education and experience may be considered in lieu of education.
  • Managerial and supervisory experience in a professional environment required
  • Experience working in the field of intellectual or developmental disabilities required
  • Experience working with nonprofit organizations preferred

REQUIRED MISCELLANEOUS QUALIFICATIONS

PHYSICAL REQUIREMENTS

  • Lift carry squat bend move push or pull sit or stand for long intermittent periods
  • Type on a keyboard and vision sufficient to view computer screens for extended periods
  • Local and intrastate driving with possible overnight stays
  • Ability and strength to position/lift/transfer an adult human being (greater than 50 lbs. from varied positionsincluding standing to laying on floor and the ability to continuously push and pull 100125 lbs. of pressure over anextended period as per the American Heart Association requirements for administering CPR.
  • Continuous ability to kneel on hard or other surfaces for extended periods as per requirements of performing CPR
