Now hiring a fulltime Discharge Planner! This position requires an active state LPN license.
East Liverpool City Hospital is an awardwinning community hospital proudly serving residents of the tristate region since 1905. With 152 licensed beds and more than 500 employees the hospital averages 31000 Emergency Department visits annually. East Liverpools medical staff is comprised of more than 160 physicians with an additional 21 resident physicians completing their training in Family Medicine and Internal Medicine. The hospital provides 24/7 emergency services general surgery medical stabilization for substance abuse behavioral health for adults over the age of 55 and remains the ONLY cardiac rehabilitation program in Columbiana County. Know Your Rights: for the coordination of the various activities of the Case Management Department under the direction of the assigned Case Manager/Social Worker assist with development and implementations of discharge plans. Assists with case management referrals and authorizations. Maintains and coordinates Medicaid state submission forms for presentation to onsite reviewer and files them accordingly or as per facilitys procedure. Assists with Discharge Planning as assigned by interviewing patients/families and gathering data to develop a safe patientfocused Discharge Plan.
1. Knowledge of Discharge Planning/ Utilization Management / Case Management terminology and functions in both managed care and nonmanaged care environments.
2. Active state LPN license required upon hire and maintain current.
3. Experience in basic to intermediate computer skills/knowledge
4. Current BCLS (AHA) certificate required upon hire and maintain current.
