GENERAL DESCRIPTION:


Responsible for general supervision oversight training and technical assistance for stakeholders and contracted providers in order to coordinate care and potential placement for children and adults experiencing a crisis. The position will oversee the implementation and management of crisis care response and provide leadership experience and technical assistance to partner organizations. The position collects maintains and analyzes qualitative data from persons experiencing a crisis their families and stakeholder agencies to address system gaps and continuously improve service quality. As a Health Program Supervisor this position manages specific activities supporting the planning design development testing piloting rollout implementation and operational phases of crisis care response to provide care coordination for individuals whose care and placement needs cross over multiple state agencies and require coordination and funding from multiple sources.

  • Assure program coordination and development through collaboration and correspondence with Division staff state agencies involved in child or adult programming and other key stakeholders. Develop and maintain positive supportive and effective relationships with key internal and external stakeholders including nonprofit entities the government health integration law enforcement and advocacy organizations. Participate in regular meetings with other crisis services personnel to coordinate cases and review intervention outcomes to enhance the provision of future emergency services.
  • Work with the Operations Policy Tracking and Contracts (OPTC) Unit Manager Operations and Projects Administrator and other division staff to establish or modify operating procedures policies objectives priorities methodologies and goals. Review and interpret the rules regulations policies and procedures surrounding this work.
  • In partnership with the appropriate guardian or legal representative complete and submit the necessary documentation to provider entities for potential admission or resource exhaustion on behalf of individuals experiencing a crisis. Assure strong service and treatment documentation auditing and followup. Produce clientrelated service records and maintain accurate and uptodate documentation consistent with agency policies and procedures. Coordinate and Facilitate multidisciplinary team meetings with appropriate parties.
  • Assure integrated service collaboration and continuity of services across state programs and the cooperation and coordination of services with other entities. Provide training and technical assistance to internal and external stakeholders on potential resources and contacts. Maintain a current list of instate and outofstate resources available and a general understanding of available funding opportunities and limitations. Update and maintain comprehensive national provider listings based on provider specialty admission criteria exclusions funding acceptance and any other information that may be pertinent to the coordination of crisis care. Utilize comprehensive listings to screen potential cases for placement or resource exhaustion.
  • Collect maintain and analyze qualitative data and information to address system gaps and continuously improve service quality. Conduct informal studies regarding the child and adult crisis resources available and make policy recommendations to key stakeholders to make meaningful programmatic change.
  • Assist in the development of biennial budgets supplemental budget requests and contract award amounts. Develop contracts and collaborative agreements as needed with key partners in cooperation with other staff. Provide administrative fiscal and oversight of all statefunded and federalgrantrelated contracts and agreements.
  • Work with applicable stakeholders to resolve Title 25 emergency detention patient billing cases.
  • Perform other duties as assigned by the supervisor which are consistent with the position and in compliance with agency policies and procedures.

Qualifications


PREFERENCES:


The most appropriate minimum education for this position is completing a bachelors work or equivalent (typically in Healthcare) in addition to 12 years of progressive work experience (typically in Health Programs) with acquired knowledge at the level of a Health Program Specialist.

46 years of progressive work experience (typically in Health Programs) with acquired knowledge at the level of a Health Program Specialist
