Developmental Disabilities Case Manager
A Great Opportunity
FullTime Monday Friday 9am5pm
$18$21 based on education and experience plusmonthly incentive program for exceeding productivity expectations

Our clients are supported by a team of mental health counselors social workers certified nurse practitioners case managers and psychiatrists to ensure the best outcomes for those we serve. Responsibilities may include:
- Provide Therapeutic Behavioral Services to clients by consulting with a licensed clinician to assist with the individuals needs.
- Provide service planning for individualized supports or care coordinator of healthcare behavioral health nonhealthcare services and development of a treatment plan.
- Provide linkage interventions treatment options restoration of daily life skills and crisis prevention services.
- Provide services to individual clients or groups of clients.
- Promote a positive and cooperative relationship with all outside contacts.
Requirements for this position include:
- High school diploma plus three or more years of relevant field experience or Bachelors or Masters level degree in a related field.
- Must be at least 18 years of age.
- Related experience or training with mental or behavioral health conditions.
- Must have a valid drivers license and reliable transportation.
- Willingness to travel throughout assigned service area.
