Essential Job Functions:
Demonstrates HRMCs Mission and Core Values of the hospital: Respectful Engaged Available Competent and Helpful and demonstrates that patients physicians employees are customers and consistently seeks to exceed expectations.
Completes a psychosocial assessment with patient family member or both within the first twentyfour hours (work week) of the admission process
Document on the day of completion with a written discharge plan that addresses use of the full continuum of care.
Collaborates closely with Social services community liaisons nursing staff and all other departments.
Collaborate with Utilization manager regarding criteria length of stay and preauthorizations.
Providers treatment updates to providers outside of the hospital and family members
Provides placement and resource assistant services
Assists in the orientation and educational programs with students and employees including the understanding of systems regulations and reimbursement of various payees.
Builds bridges/relationships with other community facilities/agencies that will benefit the hospitals operation and community planning services.
Demonstrates the knowledge and skills necessary to provide care appropriate for pediatric adolescent adult and geriatric patients.
Maintains current knowledge of available community resources for patient/family assistance and keeps appropriate nursing personnel informed of same.
Acts as an information resource person regarding Federal State and private agency review regulations and requirements.
Participates in daily rounding and discharge planning process including but not limited to the placing of patients in nursing/foster homes swing bed admission coordinating arrangements for patient/family needs and equipment for post discharge care at home along with financial assistance.
Responds to and supports urgent & emergency situations to support/assist patient families and staff.
Document each assessment and all other social visits accurately and in a timely manner.
Participates/coordinates patient/family care conferences as deemed necessary.
Prepares assessments timely in accordance to swing bed dialysis and hospice patient rules and regulations depending on social service degree held.
Provides social service activity for terminally ill patients and families including treatment planning and consultation with other staff members.
Lead/support Interdisciplinary Rounding with timely (same day) documentation.
Coordinate cares from beginning of stay thru Discharge
Other Job Duties:
Assumes responsibility for professional development. Participates in inservices internal committees educational programs mandatory & department meetings.
Complete required HealthStream education competencies certifications and SQSS tasks.
Participate in department quality risk and utilization plan. Prepare timely and accurately reports and data for the department as requested.
Promotes and maintains confidentiality of information regarding patients families health care personnel and facility.
Presents professional appearance and demeanor.
Reviews and complies with general hospital and departmental policies and procedures including infection control and universal precautions.
Works within the chain of command and performs other duties as assigned.
Qualifications:
Associates degree or equivalent experience
Bachelors degree in a healthrelated field preferred or related degree
23 years experience in a similar role
Indepth knowledge of medical terminology collecting and interpreting medical information
Demonstrates continuous patient focus within the daily responsibilities
Coordinates
Communicates congenially and professionally with patients families coworkers and physicians along with outside organizations.
Knowledge Skills and Abilities
Bilingual preferred
At least 1 year experience in community services preferred
Knowledgeable about services community needs and resources
Organized and able to work independently
Good computer Skills.
