Under the general direction of the Medical Director and the Practice Manager the Palliative Care Nurse Coordinator will work closely with the palliative care providers other providers care managers and nurses to coordinate comprehensive palliative care services. The palliative care nurse coordinator acts as a resource and support to patients families and staff for palliative care and end of life issues.
Certification Registration and Licensure Required:
Current NH RN license; Certification in Palliative Care (CHPN) within one year of hire.
Qualifications and Competencies:
Strong organizational communication and interpersonal skills.
Knowledge of palliative care principles endoflife issues and healthcare systems preferred.
Proficiency in electronic health records and Microsoft Office Suite.
Demonstrates effective time management skills and attention to detail.
Demonstrated an ability to work in a collaborative manner.
Experience:
Minimum of 5 years experience in acute care.
Responsibilities
Represent and introduce palliative care services to patients families and healthcare providers.
Utilize the nursing process to assess and facilitate care related to the specialty needs of individual patients and families for successful palliative care management and intervention.
Review daily census gather background information from referring providers and medical records to triage consult requests and assist with determining priority.
Coordinate team consult schedules and manage incoming consult requests for both inpatient and outpatient settings.
Act as a resource on palliative care and endoflife issues for patients families and staff.
Attend unit rounds when possible.
Coordinate with chaplain social work care management and hospice/home care providers (GVNA) to ensure comprehensive patient support.
Provide followup calls and bedside checkins as requested by Palliative Care providers.
Organize and assist hospitalists/intensivists in conducting family meetings in the absence of a Palliative Care Provider.
Collaborates with staff in other CHMG Palliative Care practice.
Perform data collection and data validation.
Actively participate in the Cancer Committee and other relevant committees to advocate for palliative care interests and initiatives within the organization.
