The ParentChild Assistance Program (PCAP1 is a oneyear case management intervention serving parents with atrisk alcohol and/or drug use who have children under six. The program is hiring a remote staff position in Oklahoma City. The Case Manager position is responsible for providing home visitation case management service coordination and advocacy services to these parents and their extended family as needed.

Responsibilities:

  • Perform case management activities according to PCAP1 policies and procedures in a highly confidential and professional manner.
  • Responsible for the case management process including admissions assessment/reassessment service planning monitoring and discharge of consumers. Provides support to cope with chronic or acute illnesses or conditions.
  • Advises caregivers provides patient education and counseling and makes referrals for other services. May also provide care and case management or interventions designed to promote health prevent disease and address barriers to access to healthcare/.
  • Develops and implements actionoriented and time specific case management plans by determining and documenting specific objectives goals and actions designed to meet the clients health and human service needs identified through the assessment process.
  • Gathers information and documentation from all relevant sources regarding the case management plans activities and/or services to determine the plans effectiveness in reaching desired outcomes and goals. Modifies or changes the case management plan as needed.
  • Manage a caseload of up to 20 clients/parents and conduct home visits with each client at least twice per month or as needed according to the PCAP1 model.
  • Assist clients in setting goals base on their individual needs assessments and in identifying steps to achieve their goals monitor progress.
  • Identify highquality community providers and agencies whose services are relevant to client needs.
  • Provide outreach services to engage and connect clients with community agencies that will help meet their needs and goals.
  • Transport clients to important appointments in support of their goals if needed.
  • Administer assessment instruments dealing with extremely sensitive and confidential information according to PCAP1 policies and procedures.
  • Enter client information into a secure online platform.
  • Coordinate scheduling and attend case consultation meetings and appointments with community providers with whom clients are involved.
  • Maintain regular contact with clients networks of service providers.
  • Provide housing support/case management services and documentation as required for housing subcontracts and working agreements.
  • Attend and participate in weekly individual supervision with the Project Director to discuss client needs goals and issues.
  • Participate in weekly staff meetings with the PCAP1 team to evaluate client and program progress.
  • Provide outreach services to locate and reengage clients who have disengaged from the program.
  • Participate in required trainings.
  • Obtain and maintain current CPR Infant CPR and First Aid Certification.
  • Contribute to report writing training and community presentations as needed
  • Performs other duties as needed to successfully fulfill the function of the position.

Training:

A significant amount of required training will be provided including but not limited to training on the PCAP1 model and delivery identification of child abuse and neglect mandatory reporting specific to drugendangered children infant and child mental health and Plans of Safe Care.

Individuals with lived experience dealing with substance use disorders or other relevant issues are strongly encouraged to apply.

Funding for this position is anticipated through July 2026.

Required Education: Bachelors degree in Social Work Psychology Sociology or other Health or Human Services related fieldAND:

  • 12 months experience in home or communitybased services case management or a related healthcare environment.

Skills:

  • Knowledge of federal and state laws particularly patient confidentiality.
  • Knowledge of case management fundamentals and the technical procedural aspects of case management.
  • Must have a high level of organization skills
  • Ability to resolve issues applying judgment critical thinking and problemsolving skills to determine a course of action.
  • Ability to exercise discretion when making decisions and seeks managements advice to determine proper course of action while following established standards.
  • Ability to handle stressful situations different situations or circumstances and complete projects in a timely manner.
  • Ability to keep accurate and detailed records.

  • Must pass several criminal and registry checks including a criminal background check as well as OSBI Sex Offender and Mary Rippy Violent Offender registry checks.

  • Must possess a valid drivers license and be able to drive to meet with clients in their own homes or at local community locations Must have reliable transportation (mileage is reimbursable).
  • Must pass a drivers record check in accordance with program insurance requirements.

Departmental Preferences:

  • 4 years of communitybased experience working with atrisk populations.
  • Experience working with clients/consumers/patients with substance use disorders.
  • Effective communication skills.
  • Demonstrated ability to effectively engage with atrisk parents and families.
