About the opportunity
Our Indigenous Health Project Officer (IHPO) focuses on improving access to culturally appropriate mainstream primary health care for Aboriginal communities in wide area from Augusta to Yarloop. This role will require travel and you will be provided with a vehicle for these trips. The purpose of this program is:
- Achieve better treatment and management of chronic conditions for Aboriginal and Torres Strait Islander people through better access to the required services and better care coordination and provision of supplementary services.
- Foster collaboration and support between the mainstream primary care and the Aboriginal and Torres Strait Islander health sectors.
- Improve the capacity of mainstream primary care services to deliver culturally appropriate services to Aboriginal and Torres Strait Islander people.
- Increase the uptake of Aboriginal and Torres Strait Islander specific Medicare Benefits Schedule (MBS) items including Health Assessments for Aboriginal and Torres Strait Islander people and follow up items;
- Support mainstream primary care services to encourage Aboriginal and Torres Strait Islander people to selfidentify; and increase awareness and understanding of measures relevant to mainstream primary health care.
- The Indigenous Health Project Officer works collaboratively with all other members of the SWAMS organisation in order to achieve the best possible outcomes for ITC clients.
- Provide leadership and guidance to the AHP and IOW team to ensure the ongoing provision of the highest standards of care to SWAMS clients.
Essential Criteria
- Aboriginal or Torres Strait Island decent under Section 50(d) of the Equal Opportunity Act
- Qualifications as either Aboriginal Health Practitioner Registered Nurse or Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care Practice
- Demonstrated ability to communicate effectively and sensitively with Aboriginal people.
- Ability to build stakeholder relationships
- A demonstrated understanding of the unique issues effecting and impacting upon the health of Aboriginal people.
- Knowledge and experience in the provision of health promotion programs.
- Preparedness to continue training towards increasing competencies.
- Knowledge of community and local Aboriginal cultural issues.
Desirable Criteria
- Experience and interest in Community or Aboriginal Health
- Knowledge and expertise in the use of Communicare or similar clinical database system.
