Job Description

The Contra Costa Adult Continuum of Services (CCACS) is a program of Contra Costa Health Services Health Housing and Homeless Services Division in partnership with Heluna Health. Our mission is to provide safe interim housing with comprehensive services that assist homeless adults in securing permanent housing that will end their homelessness. CCYCS

The Contra Costa Youth Continuum of Services (CCYCS) is a program of Contra Costa Health Services Health Housing and Homeless Services Division in partnership with Heluna Health. CCYCS operates with the philosophy that every young person has the ability to make good choices for themselves when provided with gentle guidance support and positive opportunities. We value the diversity determination resiliency and strength that all youth bring to our doors. ( the supervision of the CCYCS Program Manager the individual in this position will establish and maintain a caseload of up to 15 youth ages 1824. The Case Manager will provide assessments brief goaloriented counseling advocacy and support to homeless youth staying at an emergency youth shelter located in West Contra Costa County.

SHP

The Supportive Housing Programs (SHP) and Rental Assistance Programs (formerly Shelter Plus Care) are programs of Contra Costa Health Services Health Housing and Homeless Services Division in partnership with Heluna Health. Under the supervision of the Supportive Housing Manager the individual in this position will provide assessments case management services brief goaloriented counseling advocacy and housing retention services to individuals living independently. The Case Manager will be responsible for outreaching to local landlords and other housing providers in order to establish lasting working relationships to expand the housing stock available for consumers in Homeless Programs.

Salary: $24.00 $28.35 Hourly

ESSENTIAL FUNCTIONS

  • Establish and maintain a caseload and conduct Needs Assessment interviews (related to psychosocial mental and physical health housing income AOD services legal etc.; as well as VISPDAT.
  • Meet with clients weekly to assist in developing and monitoring a case/life/treatment plan
  • Assist client in obtaining appropriate identification as well as benefits such as MediCal SSI GA Cal Fresh and other benefits and services immediately upon program entry.
  • Identify support and closely coordinate clients access to resources and services related to Needs Assessment outcomes. Referrals to: Housing Specialists Income Specialists Mental Health Case Managers/Clinicians Healthcare for the Homeless Community Health Workers and Nurse Practitioners Alcohol and Other Drug Access Line/AOD Counselors legal services other referral sources as needed.
  • Support client to access community resources by transporting accompanying and coaching client when navigating community resources. Agency vehicle provided.
  • Maintain client files and associated case management documentation that include case notes documentation and electronic records on the Homeless Management Information System (HMIS).
  • Completes HMIS standardized forms. Submit completed forms to the appropriate staff member within 24hours of service.
  • Educate clients on daily living skills to improve selfsufficiency build authentic rapport to support clients goals on an ongoing basis.
  • Facilitate relationshipbuilding with clients and their family members including family reunification meeting and conflict resolution as needed
  • Regularly report and coordinate client progress during multidisciplinary team case conference.
  • Attend all required meetings trainings and case conferences.
  • Submit reports to the Program Manager as required.
  • Other duties as assigned.

CCACS Case Manager (General)

    • See Essential Functions.
  • CCACS Case Manager (Housing)
    • Meet with clients based on referral from Case Manager (General) to assist in executing timelines for individual goals and objectives leading to permanent housing.
    • Conduct and complete Housing Stability Assessments/Housing Plans.
    • Conduct outreach in the community to identify build and strengthen relationships with potential landlords and property management partners.
    • Facilitate groups/workshops (ReadytoRent Peer Support Budgeting Financial Literacy etc..
    • Facilitate housing appointments for clients to explore housing options and placement.
    • Collaborate with Income Specialist and Case Managers regarding clients individualized case plan as it relates to housing stability.
    • Update client files with associated documentation pertaining to Housing Stability Assessments/Housing Plans; as well as regularly updating Referral Tracking Log etc.
    • Complete Intake process for new clients in daily rotation with shelter staff.
    • Regularly reports and coordinates client progress to immediate Supervisor and Behavioral Health multidisciplinary team.
    • Attend required meetings and trainings.
  • CCACS Case Manager (Income)
    • Meet with clients based on referral from Case Manager (General) and assist in obtaining/ increasing income by assisting application process for General Assistance SSI SSDI SSA; OR earned income through employment.
    • Conduct and complete Income Plans.
    • Conduct outreach in the community to identify build and strengthen relationships with employment resources.
    • Refer clients to appropriate resources regarding education and employment.
    • Educate clients on budgeting and financial management in a oneonone as well as workshop capacity.
    • Collaborate with Housing Specialist and Case Managers regarding clients individualized case plan as it relates to financial stability.
    • Update client files with associated documentation such as Income Plan Financial Counseling Records; as well as regularly updating Referral Tracking Log etc.
    • Complete Intake process for new clients in daily rotation with shelter staff.
    • Regularly reports and coordinates client progress to immediate Supervisor and Behavioral Health multidisciplinary team.
    • Attend required meetings and trainings.

PROGRAM SPECIFIC

CCYCS

  • Demonstrate knowledge of the community and the condition of homeless street and runaway youth. Experience with transition age youth with complex presentations including complex trauma. Must have a strengthbased approach positive attitude and compassion for working with homeless youth.
  • Provide and document aftercare follow ups with youth after exiting the program including 30 60 90 day follow up phone calls as well as general aftercare support to high needs clients to support reintegration into the community
  • Support Shelter Specialists with providing general milieu management when necessary

CCYCS Case Manager (Housing)

  • Meet with clients based on referral from Case Manager (General) to assist in executing timelines for individual goals and objectives leading to permanent housing.
  • Conduct and complete Housing Stability Assessments/Housing Plans.
  • Conduct outreach in the community to identify build and strengthen relationships with potential landlords and property management partners.

  • Collaborate with internal Case Managers and other service providers regarding clients individualized case plan as it relates to housing stability.
  • Update client files with associated documentation pertaining to Housing Stability Assessments/Housing Plans; as well as regularly updating Referral Tracking Log etc.
  • Regularly reports and coordinates client progress to immediate Supervisor and Behavioral Health multidisciplinary team.

SHP

  • Develop relationships with local housing agencies property managers and landlords to create and identify housing options and opportunities for clients
  • Respond to community phone calls and email inquiries regarding housing resources and supportive services in Contra Costa County.
  • Periodically evaluate each residents apartment for safety hazards food and kitchen supplies.
  • Support consumers in tenant/landlord conflict resolution as required.

JOB QUALIFICATIONS

  • Possesses effective communication skills and the ability to work independently and cooperatively.
  • Thorough knowledge of homelessness mental health substance abuse HIV and cultural diversity issues.
  • Have a good knowledge of community and support service resources for special needs populations.
  • Have a strong knowledge of Motivational Interviewing Trauma Informed Care and Harm Reduction models.
  • Excellent communication skills professional demeanor sound judgment and strong organizational skills
  • Ability to work in fastpaced and highstress environments
  • A successful applicant will be fingerprinted and must pass the criteria used by the State of California to screen for criminal convictions that would be deemed detrimental to working with youth.
  • Ability to work with diverse populations.
  • Ability to communicate clearly both in verbal and written forms.
  • Good computer skills (Word Excel HMIS systems data entry Outlook) required.
  • Ability to be supportive and provide guidance in a nonjudgmental manner.
  • A valid California drivers license proof of insurance clean DMV record and reliable transportation will be needed to carry out jobrelated essential functions as travel between various locations is a requirement.
  • Required to participate in the DMV Pull Notice Program and be 21 years of age or older.

Education/Experience

  • Education: AA/AS Degree from an accredited college or university in Public Health Health Services Administration Social Work or a closely related field.
    • BA/BS in Public Health Health Services Administration Social Work or a closely related field preferred.
  • Experience: One 1 year of fulltime housing case management or its equivalent experience in a social services or mental health program providing services to homeless persons. OR
    • Substitution for Education/Experience: Three 3 years of full time or its equivalent experience working in a program serving homeless persons AND HS Diploma/GED.

Desired Qualifications:

  • Bilingual bicultural in Spanish.
  • Lived experience of homelessness and/or accessing behavioral health services.
  • Previous experience or training in street outreach and clinical case management.
  • Knowledge of Contra Costa County and community resources.
  • Knowledge of the Contra Costa emergency provider network.
  • Prior experience with documentation and billing procedures.
