Job Description
- Assures eligibility process is managed effectively and that resources are utilized in ways that support individuals and families in the best way possible.
- Process eligibility and agency referrals including collecting records/data; tracking forms; and preparing packets. Conduct COEDIs or LOC assessments. Facilitate submission of eligibility packet to Council of Government (COG) for completion of the eligibility process.
- Assist families with meeting needs including home visits explaining and completing paperwork with families.
- Complete written reports case notes/chronological entries correspondence and other written documentation as required.
- Act as a resource for families in the eligibility process providing information and making referrals to other agencies or service providers; may include attending meetings with families and assisting with paperwork/ applications.
- Review reports of individuals aging out of eligibility categories; contacts family and maintains follow up on eligibility and other referrals.
- Receives screening requests assembles needed documentation schedules and follows up on necessary assessments (e.g. C/OEDI psychological evaluations) and transmits findings to the Ohio Department of Developmental Disabilities (DODD). Completes PASRR Movement Form as required enters information into the Boards electronic filing system. (This function may be assigned to only one or more E and R team member(s); may be assigned by Community Team).
- Maintain awareness of social service agencies and community resources. Continue ongoing network with representatives including attending meetings and gathering general knowledge.
- Provide training and information regarding eligibility process and other services offered by DD to other agencies individuals in the community and DD staff.
