Job Description
Empath Health is currently seeking a Licensed Practical Nurse (LPN) to join our Continuous Care (CC) team.
8am 8pm. Three 12 hour shifts with weekend requirement days to be determined. Coverage area to include Manatee Charlotte De Soto and Sarasota County.
Under physicians orders and RN supervision the Continuous Care (CC) Licensed Practical Nurse (LPN) is responsible for the observation of patient/family needs and providing supportive care and emotional support to the patient/family unit in times of crisis in order to maximize patient comfort and enhance quality of life.
Primary tasks duties and responsibilities:
1. Under the direction of the RN case manager provides continuous bedside clinical observations prescribed care physical and emotional support to patients as directed in plan of care reporting significant changes in patient condition and family needs as needed.
2. In collaboration with the RN accepts and processes physician orders to maximize patient comfort.
3. Crisis Care nurses are crosstrained to Hospice Houses and Triage to assist with patient care needs when cases are pending.
Requirements:
- Applicants must be a graduate of an approved nursing program and own a current Florida LPN license.
- Minimum one year experience in an acute care setting providing direct patient care; previous experience as a hospice LPN preferred.
