FullTime Physical Therapist

Are you a passionate and skilled Physical Therapist seeking a fulfilling career Millennium Home Care a leading provider of inhome healthcare services is looking for a dedicated fulltime Physical Therapist to join our team. Responsibilities:

  • Patient Assessment: Conduct comprehensive evaluations of patients physical abilities limitations and functional needs to accurately diagnose impairments and develop individualized treatment plans.
  • Therapeutic Interventions: Implement a variety of therapeutic techniques including manual therapy therapeutic exercises and gait training to improve patients mobility strength balance and pain management.
  • Functional Training: Assist patients in developing compensatory strategies adaptive techniques and assistive device training to enhance their independence in activities of daily living.
  • Patient Education: Educate patients and their families on proper body mechanics home exercise programs and injury prevention strategies to promote longterm health and wellbeing.
  • Collaboration: Work collaboratively with physicians nurses occupational therapists and other healthcare professionals to develop comprehensive care plans and ensure optimal patient outcomes.
  • Documentation: Maintain accurate and timely documentation of patient evaluations treatment plans progress notes and discharge summaries.
  • Compliance: Adhere to all relevant regulatory standards and guidelines including HIPAA and Medicare regulations.

Requirements:

  • Licensure: Current and valid Physical Therapist license in the state of Florida.
  • Experience: Minimum 6 months of experience in a hospital skilled nursing facility or rehab is requested.
  • Skills: Strong clinical skills excellent communication and interpersonal abilities and a patientcentered approach to care.
  • Professionalism: Strong work ethic reliability and a commitment to providing highquality care.
  • Transportation: Reliable transportation to visit patients in their homes.
