Now Hiring: Home Health Admission RN South Park PA
Location: South Park Pennsylvania
Schedule: FridaySunday 3 Admissions Daily) 1 Admin/Flex Day (Mon or Thurs)

Join an extraordinary home health care team in Brentwood PA as a Home Health Admission RN in a vital role where youll manage new patient admissions create personalized care plans and help individuals heal where they feel safest at home.
Required Qualifications:
-
Associate Degree or higher in Nursing
-
Active RN License in Pennsylvania (PA)
-
Valid Drivers License and reliable transportation
-
At least 1 year of experience with:
-
EMR / EHR systems
-
Practice Partner EMR or equivalent
-
-
One of the following certifications:
-
AHA Basic Life Support (BLS)
-
AHA Advanced Cardiac Life Support (ACLS)
-
Preferred:
-

Strong time management and communication skills

Experience with home health admissions and charting
Key Responsibilities:
-
Conduct three patient admissions per day (FriSun)
-
Build and implement individualized care plans
-
Collaborate with clinical teams for seamless transitions
-
Use mobile tools and EMR to chart and document care
-
Manage your time independently and meet documentation deadlines
-
Serve as a key liaison between patients families and care teams
Schedule Overview:

FridaySunday: 3 patient admissions per day
-
1 Flex/Admin Day/Week (preferably Monday or Thursday)
