Home Health Admission Registered Nurse (RN) Irwin PA
Location: Irwin Pennsylvania
Schedule: MonFri 8Hour Days Light OnCall Rotation

Were hiring a Home Health Admission RN to join a collaborative missiondriven care team in Irwin PA. Youll help patients transition into care craft personalized care plans and play a pivotal role in their healing journey all while maintaining autonomy and flexibility.
Minimum Qualifications:

Associate degree or higher in Nursing
-
Active RN License in Pennsylvania (PA)
-
Valid Drivers License and reliable transportation
-
1 years of EMR experience including:
-
Electronic Medical Record (EMR)
-
Practice Partner EMR
-
-
One of the following certifications:
-
American Heart Association BLS
-
American Heart Association ACLS
-
Preferred:
-
RN License number submitted with application
-
Familiarity with home health protocols
-
Selfdirected with excellent communication and organizational skills
Key Responsibilities:
-
Manage and deliver highquality home health admission care plans
-
Coordinate with the care team to ensure smooth patient transitions
-
Use tablets mobile phones and EMR software for accurate documentation
-
Meet critical deadlines for charting and followups
-
Balance your day with autonomy flexibility and compassionate care
-
Act as the point of contact for new patients and their families
Schedule:
-
MondayFriday 8Hour Shifts
-
1 evening oncall per week 5 PM 10 PM)
-
1 weekend of call per month (Sat/Sun)
