Job Description
Overview
Were hiring a compassionate selfmotivated Physical Therapist to join a toptier home health team serving patients across Lansing Michigan. In this role youll deliver patientcentered care in the comfort of patients homes supported by dedicated technology flexible scheduling and career development resources.
Key Responsibilities
- Provide skilled physical therapy treatment to home health patients
- Collaborate with interdisciplinary care teams to implement patient care plans
- Maintain accurate timely clinical documentation in the EMR system
- Communicate with patients families and other healthcare providers
- Utilize companyprovided tablet and smartphone to complete documentation efficiently
Required Qualifications
- Education: Bachelors degree or higher in Physical Therapy from an accredited institution
- License: Valid Michigan Physical Therapist license
- Experience:
- 1 years of healthcare experience
- 1 years of experience with any of the following:
- Electronic Medical Records (EMR)
- TruChart EHR or other EMR systems
- Certifications: Valid Drivers License (DL)
- Must not have applied to or worked for Residential Home Health & Hospice in the past 6 months
- Must have spoken with a Relode recruiter about this position
Preferred
