Job Summary:

Responsible for assuring the timely completion of accurate assessments and interdisciplinary care plans that meet Federal and State guidelines. Included is identifying resident acuity and needs helping to determine specific care needs and communicating needs/expectations to families and responsible parties. Overall responsibility of resident care and fiscal management of the nursing department. Is proactive in the development and implementation of facility practices to ensure regulatory compliance related to Resident Assessment Instrument (RAI) processes.

Work Schedule: Monday through Friday 8:00 a.m. to 4:30 p.m. routinely. Flexibility appreciated.

Responsibilities:

Essential Functions

Adheres to the MDS RAI Manual in coding and care planning of the Comprehensive MDS Assessment.

Assesses information and knowledge about residents for accurate completion of the MDS.

Determines the severity functional impact and scope of a residents problems; understanding the causes and relationships between a residents problems.

Establishes a course of action that moves a resident toward a specific goal utilizing individual resident strengths and interdisciplinary expertise.

Implements the care plan by staff knowledge about resident care goals and approaches.

Evaluates care plan goals interventions and implementations in terms of achieved resident outcomes and assessing the need to modify the care plan to adjust to changes in the residents status either improvement or decline.

Formulates nursing diagnoses to identify and plan for the needs of an individual family or group to ensure interdisciplinary approach to resident cares.

Utilizes history and Resident Assessment information to identify resident problems and set goals.

Demonstrates the ability to revise plan of care as indicated by residents response to treatment or physical changes occurring.

Utilizes assessment data and formulates a plan for discharge needs.

Communicates information regarding MDS/RAPS (Resident Assessment Protocols) appropriately and clearly to all disciplines involved in the residents care.

Implementation of Omnibus Budget Reconciliation Act (OBRA) and Prospective Payment System (PPS) assessment schedules. Possesses the OBRA and PPS knowledge and skills required to ensure compliance with timing and scheduling requirements for MDS Comprehensive Assessments.

Completes all required scheduled and unscheduled MDS assessments according to regulatory timeline.

Completes signature and dating of MDS according to regulatory guidelines.

Evaluates the plan of care and treatment for all residents under his/her care to promote resolution of resident problems and positive resident outcomes.

Effectively communicates between disciplines about resident care plans and needs. Routinely involves resident/family in review of resident care plans.

Coordinates the completion and submission of the Resident Assessment Instrument to ensure accurate transmission of information to regulatory agencies and compliance of facility to government standards.

Knowledge and skills of MDS completion encoding and transmitting.

Checks accuracy of MDS codings as appropriate to provide for facility compliance and proper PPS billing. Reviews all final validation reports for fatal errors and nonfatal errors.

Job Requirements

Education

  • Graduated from an accredited Diploma School of Nursing/College of Nursing with an Associates of Science Degree in Nursing (ASN) or a Bachelor of Science in Nursing (BSN) required.


License/Certifications

  • Current valid Registered Nurse (RN) license valid compact multistate license or a temporary permit while awaiting licensure required.
  • Current American Heart Association Basic Life Support (BLS) strongly preferred at time of hire required within 3 months of hire.
  • MDS (Minimum Data Set) Coordinator Certification preferred.

Experience

  • Three years of experience in Long Term Care (LTC) required.
  • Three to Five years previous MDS experience preferred.


Skills

  • Resident Assessment Instrument training preferred.
