Client Service:
Assist and provide treatment goal development per the treatment plan and according to the health care needs of clients and assist in the development of health care goals that will allow them to achieve the greatest level of independence in their community.
Share information with the Interdisciplinary Team regarding client goal progress significant events and/or client responses that vary from their baseline as it relates to their treatment plan.
Review monitor and facilitate treatment options for clients health needs. Report outcomes to Interdisciplinary Team within established guidelines.
Ensure medical office area maintains adequate medical and office supplies.
Ensure all residential locations maintain first aid kits and emergency medical supplies as defined by Iowa code and accrediting bodies.
Utilize critical thinking in making independent judgements and maintain responsibility and accountability for service provided.
Knowledge of the effects of mental illness and the ability to relate respectfully to clients with a wide range of abilities and diagnoses.
Possess teamwork skills including problem solving cooperation negotiating etc.
Continue professional development and involvement in professional activities that enhance service provision.
Monitor train and review medication management administration by direct supports. Collaborate with Supervisors in supervision of medication administration and management by direct supports.
Work directly with client and Eyerly Ball pharmacies to ensure accurate ordering and distribution of medications.
Ensure all client medical documentation is maintained per established Department of Inspections and Appeals CARF and other accrediting body guidelines.
Schedule and oversee client medical and psychiatric appointments.
Follow up on medical and psychiatric appointments to ensure accurate implementation and completion of physician orders.
Facilitate and maintain professional relationships with medical professionals.
Work closely and under the direction with Residential RN on tasks that need to be completed or followup care as needed in the evening hours and weekends as needed.
Documentation:
Complete all documentation within established guidelines.
Monitor direct support staff medication administration to ensure it meets established guidelines.
Ability to prioritize client need and to complete required written documentation.
Demonstrate an understanding of all funder and accreditation standards.
Assist Supervisors with obtaining and reviewing client admission and discharge information.
Ensure all client medical requirements are documented per established DIA CARF and other accrediting guidelines.
Oversee all aspects of eMAR.
May support other areas of Eyerly Ball as needed.
Be available at minimum for PT hours and in the evening hours and weekends.
Education: LPN Nursing degree required from accredited college or CMA. Human Services or related field.
Experience: Two years of experience working in the behavioral health field preferred.
License(s)/Certification(s): Valid drivers license when driving any vehicle for workrelated reasons. Adult CPR First Aid and Mandatory Dependent Adult Abuse certification.
Knowledge/Skills/Abilities: Ability to speak read and write fluent English. Ability to read and comprehend common terminology of the mental health field.
Ability to effectively present information and respond to questions from clients medical professionals management staff and the public.
Other: Must be flexible with scheduling to include evening hours and weekend hours.
