Hours of typical operation are MF 7:00am 4:00pm during training. Must be able to work a flexible schedule as needed.
Benefited position coverage eligibility will begin 1st of the month following date of hire.
Position Summary:
The Clinical Pharmacist is responsible for the provision of safe effective and prompt medication therapy to District patients. Provide support of medicationuse systems and deliver optimal medication therapy to patients with a broad range of disease states provide direct patient care in coordination with physicians nurses and other hospital personnel. These clinicians are aligned with interdisciplinary programs and specialty services to deliver medication therapy and to ensure pharmaceutical care programs are appropriately integrated throughout the hospital. Participate in all necessary aspects of the medicationuse system while providing comprehensive and individualized pharmaceutical care to patients. Pharmaceutical care services include assessing patient needs incorporating age and disease specific characteristics into drug therapy and patient education adjusting care according to patient response and providing clinical interventions to detect mitigate and prevent medication adverse events. Serve as departmental resources to other departments District personnel or external groups as well as conduct patient care quality and regulatory compliance initiatives designed to improve medicationuse processes or pharmacy practice. Provide medication and practicerelated education/training. Participation in the quality management program is expected with the goal of improving pharmacy services. Responsibilities may include working to establish and maintain long and short term goals for the Quality Management Program; monitoring and documenting Quality Improvement Projects for progress in meeting QI goals; and providing guidance and education to staff on Quality Management projects.
Requirements
Education:
Graduate of an ACPE accredited College of Pharmacy.
Licensure/Certification/Registration:
Licensed (or eligible for licensure) in the state of Oregon
Experience:
Clinical pharmacy experience in a hospital setting preferred.
