This position under the direction of a Registered Nurse participates in direct and indirect patient care and health teaching to the full extent of his/her formal education continuing education competence and experience. Patient care may range from the newborn through the geriatric stage of life. May delegate duties to other caregivers as required and/or permitted and will consistently provide appropriate supervision. Works with other staff members in order to meet the goals objectives and standards of the department and contributes to the attainment of divisional and system wide goals and objectives. Continues to add to his/her nursing knowledge and skill through ongoing education.

LPN Home Health and Hospice

I. Major Responsibilities:
1. Provides patient care according to standards of practice and departmental policy and procedure.
2. Provides assistance in patient education and documents appropriately according to policy and procedure.
3. Achieves continuity of patient care through planning and exchange of information with other health care professionals.
4. Administers medications safely and in accordance with professional standards of practice and departmental policies and procedures within the scope of practice for the LPN.
5. Makes recommendations for changes to the plan of care to the appropriate RN or other team member as the patient condition warrants.
6. Delegates patient care activities to appropriate personnel in accordance with professional standards of practice and departmental policy and procedure and within the scope of practice.
7. Meets productivity guidelines 7 visits per 8hour day) according to departmental policy and procedures.
8. Supports performance improvement initiatives through active participation in PI processes projects and teams.
9. Creates and maintains documentation which is consistently complete accurate timely and legible in accordance with departmental policies and procedures.
10. Maintains a safe environment for our patients through consistent compliance with policies and procedures.
11. Possesses and applies the skills and knowledge necessary to provide care to patients and customers throughout the life span with consideration of aging processes human development stages and cultural patterns in each step of the care process.

1. Complies with established departmental policies procedures and objectives.
2. Attends variety of meetings conferences seminars as required or directed.
3. Demonstrates use of Quality Improvement in daily operations.
4. Complies with all health and safety regulations and requirements.
5. Respects diverse views and approaches demonstrates Standards of Respect and contributes to creating and maintaining an environment of professionalism tolerance civility and acceptance toward all employees patients and visitors.
6. Maintains regular reliable and predictable attendance.
7. Performs other similar and related duties as required or directed.

II. Position Qualifications:

License/Certification/Education:
Required:

1. Current licensure as a Licensed Practical Nurse in the Commonwealth of Massachusetts.
2. Successful completion of an accredited Licensed Practical Nurse Program.
3. Valid USissued drivers license.
4. Registered inspected and insured automobile for work related purposes.


Preferred:
1. Current Basic Life Support (BLS) certification.
2. Current CPR certification.


Experience/Skills:
Required:


1. Ability to perform detailed assignments with minimal supervision and work under pressure.
2. Knowledge of the nursing process for the delivery and management of patient care.
3. Ability to create electronic documentation which is accurate intelligible and concise.
4. Ability to complete and maintain all required licensed practical nurses competencies.
5. Ability to communicate proactively and clearly with patients caregivers and medical staff.
6. Ability to read write and speak clearly in English.


Preferred:

1. Minimum two 2 years of MedicalSurgical nursing experience in direct patient care.


Unless certification licensure or registration is required an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.

Home Health Aide

I. Major Responsibilities:
1. Provides direct and indirect personal care in a safe and timely manner in accordance with departmental policies and procedures.
2. Performs homemaking and other environmental services in a way that facilitates sanitary and safe care in the patients home according to policies and procedures.

3. Duties may include:
a. Providing patients with mobility assistance bathing dressing and grooming.
b. Providing patients and families with emotional support and instruction in areas such as caring for patient or self preparing healthy meals living independently or adapting to disability or illness.
c. Changing bed linens washing patients laundry cleaning patients living area and other homemaking tasks identified within the home health plan of care.
d. Preparing or serving meals to patients according to the home health plan of care.
e. Directing patients in simple prescribed exercises or in the use of braces or artificial limbs.
f. Checking patients pulse temperature and respiration.
g. Changing clean dressings.
h. Performing a variety of duties as outlined in the home health plan of care.

4. Maintains records of patient care condition progress or problems to report and discusses observations with supervisor or professional according to policy.
5. Observes documents and reports changes in client status or expectations of services promptly to a supervisor or professional according to policy.
6. Recognizes emergency situations and implements interventions according to appropriate departmental procedures and policies.
7. Renders services in accordance with the written plan of care as documented by the appropriate professional.
