A DAY IN THE LIFE OF A LEAD CARE NAVIGATOR
As a Care Navigator you will work with the grant team to determine the internal referral process for three services: evidencebased dementia intervention dementiaspecific care navigation and expert consultation for people living with dementia and/or their care partners. You will build referral partnerships educate Trellis staff on services and provide dementia education to potential referral gatekeepers. You will deliver EB/EI intervention to family caregivers and persons living with dementia including persons with dementia living alone as well as dementiaspecific care navigation and expert consultation. Along with the project team you will assess evaluation results on an ongoing basis to assess the viability and sustainability of grant activities.
WORK SCHEDULE
This hybrid position is 30 hours a week working in the community at home and in the office Monday Friday 8:00 AM4:30 PM.
QUALIFICATIONS
The ideal candidate is an experienced public speaker with strong communication customer service and problemsolving skills. They are strategic manage time well and can handle multiple tasks and responsibilities. They are skilled in interviewing clients conducting personcentered assessments and have a thorough understanding of community resources public assistance programs and Medicare.
